Opere midelerde sintigrafik yöntemle gösterilen duodenogastrik reflüde Helicobacter pylori prevalansı

Özet
Parsiyel gastrektomisi ve gastroenterostomisi olan hastalarda, safra reflusuna bağlı olarak spesifik bir gastrit gelişir. Peptik ülser nedeniyle böyle bir ameliyat geçiren hastalarda Helicobacter pylori (H. pylori) preva-lansının da yüksek olması beklenir. Ancak postoperatif safra reflusu olan hastalarda H. pylori prevalansi %20-60 arasında tesbit edilmiş ve duodenogastrik reftü (DGR)nun safra asitleri nedeniyle H. pylori infeksiyonu-nu azalttığı ileri sürülmüştür. Bu çalışmanın amacı DGR'nun H. pylori prevalansına etkisini araştırmaktır. Bu amaçla, peptik ülser nedeniyle parsiyel gastrektomi ve gastroenterostomi ameliyatı olmuş 22 hasta çalışma¬ya alındı. Semptomlar sorgulandıktan sonra endoskopi yapılarak safra reflusu incelendi. Mide mukozasından çok sayıda alınan biyopsi spesmenleri, histopatolojik yöntemlerle gastrit ve H. pylori yönünden tetkik edildi. Daha sonra hastalara Tc 99m EHIDA hepatobilier sin-tigrafi uygulandı ve enterogastrik reflü indexi bulundu. Indexi %57'den büyük olan 9 hastaya DGR tanısı kondu. Sonuçlar korelasyon matrix yöntemiyle incelendi. DGR'u olan ve olmayan (22 hasta) hastalarda yaş, cinsiyet, semptomlar, endoskopik tanı, gastrit oranı, operasyon nedeni, şekli ve zamanı benzerdi. DGR ve H. pylori ile semptom, endoskopik bulgu ve gastrit arasında ilişki saptanmadı. DGR'u olanlarda H. pylori pozitifliği %46.15 iken, olmayanlarda %88.89 bulundu (p= 0.021). Reflü derecesi ile H. pylori pozitifliği arasında da negatif bir korelasyon vardı (r= -0.4628, p<0.05). Sonuç olarak DGR, opere midelerde H. pylori prevalansını belirgin olarak azaltır.
Abstract
Patients who have partial gastrectomies and gastroenterostomies develop a specific histologic gastritis attributed to reflux of bile. Patients undergoing these operations for peptic ulcer disease would be expected to have a high prevalence of Helicobacter pylori (H. pylori). However, prevalence ofH. pylori has been found between 20% and 60% in the patients with reflux of bile after gast-roenterostomy and it has been suggested that reflux of duodenal contents, including bile acids, decreases H. pylori infection. The aim of this study was to investigate the effect of duodenogastric reflux (DGR) on H. pylori 22 patients who have partial gastrectomies and gastroente¬rostomies because of peptic ulcer were studied. Existence of bile reflux was observed with endoscope after a questi¬onnaire was administered to the patients about their complaints. H. pylori and gastritis were identified by his-tological tests in biopsy specimens of gastric mucosa. Tc 99m EHIDA hepatobiliary scintigraphy was performed to the patients and enterogastric reflux index was deter¬mined. Nine patients whom indexes were higher than 57% diagnosed as DGR. Results were compared with matrix correlation. The age, sex, complaints, endoscopic diagnosis, rate of gastritis and characteristics of operati¬on were comparable in patients with DGR or not (13 pa¬tients). DGR and H. pylori were not associated with complaints, endoscopic findings and gastritis, v was fo¬und as 46.15% in patients with DGR and 88.89% in ot¬hers (p= 0.021). There was a negative correlation with degree of reflux and H. pylori (r= -0.4628, p<0.05). In conclusion, DGR reduces the prevalence of H. pylori in operated stomachs, significantly.
