Güneydoğu Anadolu Bölgesinde karaciğer sirozu vakalarında hepatosellüler karsinoma sıklığı

Özet
Bu çalışmada Güneydoğu Anadolu Bölgesinde 156 karaciğer sirozu vakasında hepatoselüler karsinoma (HSK) sıklığı araştırıldı. Ortalama yaş 47, 9 kadın erkek oranı 112.2 idi. Vakalar ultrasonografi (USG), alkalen fosfatoz (ALP), alfafetoprotein (AFP) ve ferritin düzeyleri yönünden araştırıldı. HSK şüpheli hastalarda bilgisayarlı tomografi (BT) yapıldı ve karaciğer biopsisi ile tanı doğrulandı. 156 vakanın 8 inde (% 5.1) HSK tespit edildi. Bunların 1'i Child B, 7 si Child C idi, ALP, AFP ve ferritin düzeyleri anlamlı şekilde yüksekti (p<0.001). Vakaların hepsi erkek ve ortalama yaş 56.2 idi. Bütün vakalarda HBsAg müsbet, 1 vakada HBsAg ile beraber Anti HCV, 1 vakada da Anti HDV müspetti. Tümör tüm vakalarda sağ lobda görüldü, l vakada diffüz, 7 vakada ise 2-7 cm. lik lezyon şeklinde idi. Sonuç olarak, bölgemizde karaciğer sirozu sık görülmesine rağmen HSK oranı düşüktür(% 5.1). Yaşam süresinin kısalığı, komplikasyonların ağırlığı veya viral ajanın genetik özelliği önemli olabilir. ALP, AFP, ferritin düzeyleri ve USG takipleri değerlidir. HBV karaciğer sirozu ve HSK vakalarında en başta gelen ajandır.
Abstract
In this study, the prevalence of hepatocellular carcinoma (HCC) in patients liver cirrhosis in Southeastern Turkey was been investigated. Median age was 47.9 years and female/male ratio was 1/2.2. Ultrasonography (USG), alkalene phosphatose (ALP), alpha fetoprotein (AFP) and ferritin were studied in all cases; CT was perfomed in suspected cases and HCC was diagnosed by liver biopsy. HCC was found in eight out of 156 patients with liver cirrhosis (5.1%). One of the HCC cases was in Child B, and the others in Child C. Levels of ALP, AFP and ferritin were significantly high (p<0.001). All cases were male and the average age was 56.2. HBsAg was positive in all cases. In addition anti-HCV and anti-HDV were positive in two patients. HCC was located in the right lobe in all cases. It was localized type (2 to 7 cm) in seven cases and diffuse type in one case. In conclusion, the prevalence of HCC seems to be lower than expected (5.1% ) in our region in spite of the high frequency of liver cirrhosis. The short survival period, severity of complications and genetic properties of viral agents may be important. The levels of ALP, AFP and ferritin and examination of USG are important in follow up of patients. Hepatitis B virüs is the most important factor not only in liver cirrhosis, but also in HCC cases in our region.
