Özet

Üst sindirim sistemi endoskopisi (ÜSE) günümüz¬de çok sık kullanılan bir tanı aracı olması nedeniyle ba¬zı riskleri gözden kaçmaktadır. Bu çalışmamızda en-doskopi sebebi ile bakteriyemi oranının beklenenden fazla oranda olabileceğini göstermeyi amaçladık. Ayrıca bakteriyemi ile ilişkili olabilecek risk faktörlerini araş¬tırdık. Çeşitli sebepler ile 54 hastaya randomize olarak üst sedirim sistemi endoskopisi uygulandı. Endoskopi öncesi ve sonrası her hastanın antekübital veninden 4ml kan kültür örneği alındı. Hastaların %65'inde endosko¬pi sonrası kan kültüründe üreme tesbit edildi. En sık üretilen (12 hasta) proteus spp'in musluk suyu ve endos-kop üzerinde tesbit edilmesi, bakteriyemi oranının yük¬sekliğinin dışarıdan kaynaklandığını düşündürdü. Çe¬şitli klinik ve biyokimyasal risk faktörleri incelendiğin¬de (lojistik regresyon analizi ile) hiçbirisinin bakteriye-miyi artıran bir etkisinin olmadığı tesbit edildi. Sonuç¬ta endoskopi sırasında yüksek oranda kana bakteri ge¬çişi olduğu ve bunun çevre koşullarına bağlı olduğu, hastaya bağlı kinik ve biyokimyasal parametrelerin bakteriyemiyi etkilemediği düşünüldü.

Abstract

Due to the increasing use of the upper gast¬rointestinal endoscopy (UGE), some of the risks of this technique may be overlooked. In this clinical study, we aimed to show that the bacteremia rate after UGE is more than expected. We also investigated some risk fac¬tors that could be related to bacteremia. UGE was per¬formed to 54 andomly selected patients with different aetiologies. Four ml of blood culture sample was drawn from antecubital vein in each patient before and after endoscopy. 65% of the patients had bacteremia after en¬doscopy.Proteus spp that was must frequently seen in blood cultures (12 pts) was also cultured from the tip of the endoscope and tap water. This showed us high inci¬dence of bacteremia was due to environmental contami¬nation. After investigation of the diverse clinical and bi¬ochemical risk factors with logistic regression analysis, none of them was related with bacteremia. As a conclu¬sion, high rate of bacteremia occurs during the UGE in relation with the environmental factors not due to the patient's clinical status and biochemical parameters.
