Duodenal ülserli hastalarda gastrit tipi, lokalizasyonu ve helicobacter pylori ile ilişkisi

Özet

Duodenal ülser hastalığı ile kronik aktif antral gastrit arasında belirgin bir ilişki bulunmaktadır. Duodenal ülserli hastaların % 95 inde kronik aktif antral gastrit vardır. Bu hastaların mide korpusunda ise gastrit oranı % 25 civarındadır. Bu çalışmanın amacı, duodenal ülserli hastaların mide korpus ve antrumundaki gastrit tipini, Helicobacter pylori (H.pylori) dağılımını ve gastrit tipi ile arasındaki ilişkiyi incelemektir. Bu amaçla, tedavi almayan 70 duodenal ülserli hasta (39 E, 31 K; 37.79±11.37 yaş) çalışmaya alındı. Proksimal korpustan ve antrumdan alınan biyopsi spesimenleri, mukozal inflamasyonu ve H.pylori'yi göstermek için sırasıyla Hematoxylin-eosin ve Warthin-starry ile boyanarak incelendi. Korpusta % 65.72, antrumda ise % 98.57 oranında gastrit bulundu. Gastrit tipi ve gastritin ile ilişkisi, korpus ve antrumda belirgin farklılıklar gösteriyordu. Korpusta nonaktif (% 61.43) ve superfisiyel (% 54.29) gastrit ön planda iken, antrumda aktif (% 85.71) ve atrofik (% 78.57) gastrit ön plandaydı (p<0.001). Korpustaki (% 42.86) ile antrumdaki (% 85.71) ilişkili bulundu (r=0.271, p<0.05). korpusta superfisiyel, antrumda ise aktif atrofik gastritle birlikteydi. Sonuç olarak, duodenal ülserli hastalarda pangastrit oranı yüksek bulunmakla birlikte, daha ziyade antrumda aktif gastrite yol açmaktadır. Bu gözlem duodenal ülserli hastalarda, korpustaki inflamasyonun ile ilişkili olmayabileceğini gösterir.

Abstract

There is a strong association between duodenal ulcer disease and chronic active antral gastritis. More than 95% of patients with duodenal ulcer are found to have chronic active antral gastritis. Gastritis of the body occurs much less commonly, in about 25% of patients. The aim of this study was to investigate the degree of gastritis, the distribution of Helicobacter pylori (H.pylori), and the association of H.pylori with the degree of gastritis in the body and antrum of duodenal ulcer patients. 70 patients with duodenal ulcer (39 M, 31 F; 37.79+11.37 yrs) were studied. Biopsy specimens taken from the gastric corpus and antrum were stained with hematoxylin-eosin and warthin-starry to test for mucosal inflammation and H.pylori, respectively. Antral gastritis occurred in 98.57% and gastritis of the body occurred in 65.72% of the patients. The degree of gastritis and relationship to H.pylori was different in the body and antrum. Non-active (61.43% ) and superficial (54.29% ) gastritis was predominant in the body, but active (85.71% ) and atrophic (78.57% ) gastritis was predominant in antrum (p<0.001). A good correlation was found between the presence of H.pylori in the corpus (42.86% ) and antrum (85.71% ) (r=0.271, p<0.005). H.pylori was associated with superficial gastritis in the corpus and with active atrophic gastritis in the antrum. In conclusion, although pan-gastritis was found to be high in duodenal ulcer patients, H.pylori was associated with active gastritis in the antrum rather than the gastric body. This observation indicates that the occurrence of corporal inflammation in duodenal ulcer patients may not be related to H.pylori.
