Karaciğer sirozunda serum nitrik oksit ve kan basıncı ile ilişkisi

Özet
Bu çalışmada karaciğer sirozunda görülen hiperdinamik dolaşımda nitrik oksit'in (NO) rolünü irdelemek amacıyla dekompanse karaciğer sirozlu 40 hasta (Grup 1), kompanse karaci¤er sirozlu 10 hasta (Grup 2) ve sağlıklı 20 kiğide (Grup 3) serum NO seviyesinin göstergesi olan NO2 ve NO3 seviyeleri tespit edilerek, serum NO seviyesi ile arteryel kan basıncı arasında ilişki olup olmadığı araştırıldı. Ortalama serum NO düzeyleri grup 1'de 18.8 ± nmol / mL, grup 2'de 10.44 ± 2.89 nmol/mL idi; aralarındaki fark istatistiksel olarak anlamlıydı (p<0.001). Kontrol grubunda ortalama serum NO düzeyi 2.87 ± 1.08 nmol/mL idi. Hem grup 1, hem de grup 2'de serum NO düzeyleri kontrol grubundan yüksekti (p<0.001). Kompanse sirozlu olgularda serum nitrik düzeyleri ile sistolik, diastolik ve ortalama kan basınçları arasında anlamlı korelasyon varken (sırasıyla r=-0.68, p<0.05; r= - 0.94, p<0.0001 ve r=- 0.78, p<0.01); dekompanse sirozlu olgularda serum nitrik düzeyleri ile sistolik ve ortalama kan basınçları arasında korelasyon anlamlı bulundu (sırasıyla- 0.83, p<0.0001 ve r= -0.88, p<0.0001). Bu çalışmada karaciğer sirozlu hastalarda serum NO seviyesinin sağlıklı kişilerden daha yüksek olduğu, karaciğer sirozunda NO sentezi ve dolaşıma salınımındaki artışın bu olgularda kan basıncı düşüklüğü ile ilişkili olabileceği sonucuna varıldı.
Abstract
In this study, in order to investigate the role of nitric oxide (NO) in liver cirrhosis, in which hyperdinamic circulation is commonly seen, serum NO2 and NO3 levels, as indicators of serum NO level, were measured in 40 cases with decompansated liver cirrhosis (group 1), 10 cases with compansated liver cirrhosis (group 2) and 20 healthy subjects (group 3), and the relation between the nitric oxide and arterial blood pressure were also investigated. Mean serum NO levels were 18.8 ± nmol/mL and 10.44 ± 2.89 nmol/mL in group 1 and 2, respectively; the difference was statistically significant (p < 0.001). Mean serum NO level was found as 2.87 ± 1.08 nmol/mL in the control group. Serum NO levels were significantly correlated both systolic and mean arterial blood pressure in subjects with decompansated liver cirrhosis (r= - 0.68, p<0.0001; r= -0.94, p<0.0001 respectively ), whereas the NO levels in subjects with compansated liver cirrhosis were significantly correlated among systolic , diastolic and mean arterial blood pressure (r= -0.68, p< 0.05; r= -0.94, p< 0.001; r= -0.78, p< 0.01 respectively). This study showed that cirrhotic patients had higher serum levels of NO than those of the controls, and it has been concluded that in the case of liver cirrhosis elevated level of NO and its release into the circulation might be associated with decreased blood pressure.
