The Sydney System in the assessment of gastritis: Inter-observer agreement

Özet
Amaç: Bu çalışmada gözlemciler arası uyum araştırılarak, Sydney Sistemi' nin rutin uygulanabilirliğinin değerlendirilmesi amaçlanmıştır. Yöntem:Bir yıl içinde rutin inceleme sırasında kronik gastrit tanısı almış, en az bir korpus, bir antrom tam kat mukoza örneği içeren 50 gastrik biyopsi materyalinin Hematoksilen ve eozin kesitleri ve doku Giemzası ile boyanmış kesitleri , üç gastrointestinal sistem patoloğu tarafından revize edilmiş Sydeny Sistemi' ne göre değerlendirilmiştir. Bu değerlendirmede korpus ve antrum mukoza biyopsileri; kronik inflamasyon, aktivite, atrofi, intestinal metaplazi ve Helicobacter pylori yoğınluğu açısından 0-3 arasında sınıflandırılmıştır. Gözlemciler arasındaki uyum ölçümü "ikiden fazla gözlemci arasındaki uyum ölçümü testi" ve gözlemcilerin ikişerli uyum ölçümü ise kappa testi ile değerlendirilmiştir. Bulgular: Gözlemcilerin değişik histopatolojik parametreler için elde ettikleri tam uyum yüzdeleri %22 (korpus mukozası için atrofi)- %96 (korpus mukozası için intestinal metaplazi) iken ikişerli uyum yüzdeleri %34 (korpus mukozası için atrofi)-%98 (antrum mukozası için intestinal metaplazi) arasında değişmekteydi. Sonuç:Sonuçlar her ne kadar gözlemciler arasında zayıf-orta uyumu gösterse de ilerisi için umut veren bu sınıflamanın rutinde uygulanabilme potansiyeli taşıdığı ve uygulama sırasında karşılaşılan sorunların deneyim ve zamanla üstesinden gelineceğine inanmaktayız.
Abstract
Backgorund/aims: The aim of this study was to determine the level of inter-observer agreement between pathologists in the application of the Sydney System and to find out whether this grading system is reproduciblein routine histopathological practice. Methods: Hematoxylin and eosin tissue Giemsa stained sections of 50 gastric biopsy specimens were evaluated. These were full thickness mucosa of at least one corpus and one antral biopsy, diagnosed as chronic gastrits during routine histopathological examination within the previous year and they were gastrointestinal pathologists. Each observer graded chronic inflammation, activity, atrphy, intestibal metaplasia and Helicobacter pylori density in the corpus and antrum on a scale 0-3. The measurement of agreement among observers and the pairwise agreement on the histopathological grades were examined by "measures of agreement among kraters" and happa statistics respectively. Results: While the porportion of overall agreement on the various features by the observers ranged from 22% for atrophy in the corupus to 96% for intestinal metaplasia in the corpus, that of pairwise agreement ranged form 34% for atrophy in the corpus to 98% for antral intestinal metaplasia. Conclusions: Although the results of this study showed poor to moderate inter-observer agreement, we believe that thşs promising classification has a potential value in routine practice and that the problems faced during application of the system will be overcome by time and experience.
