Özet
Amaç: Polikistik over sendromlu (PCOS) hastalarda izlenen kilo artışı ve hiperinsülinemi gibi pek çok problem tedaviyi daha kompleks bir duruma getirmektedir. PCOS’lu hastalar infertilite tedavisi öncesinde, infertilitenin diğer sebepleri, hiperinsülinemi ve genel sağlık sorunları yönünden detaylı olarak değerlendirilmelidir. In vitro fertilizasyon (IVF) öncesi over stimülasyonunda ciddi hiperstimülasyon riski önleyici yaklaşımlarla kontrol edilmelidir. Hafif stimülasyon uygulanan ya da uygulanmayan siklüslerden elde edilen oositlerin in vitro matürasyonu (IVM) düşünülmesi gereken geçerli bir tedavi seçeneğini temsil etmektedir. Bu derlemede PCOS’lu hastalarda over stimülasyonundaki tedavi seçenekleri ve güncel yaklaşımlar özetlenmektedir.
Abstract
Patients with PCOS demonstrate many problems, such as excessive body weight and hyperinsulinaemia, that render management more complex. Prior to treatment for infertility, the PCOS patient should be thoroughly evaluated for other causes of infertility, hyperinsulinaemia, and other general health related problems. Ovarian stimulation for IVF carries the risk of severe hyperstimulation, which should be controlled in most cases with preventive measures. In-vitro ma¬turation (IVM) of oocytes retrieved from non-stimulated or minimally stimulated cycles represents a viable option that could be considered . The current review summarizes the therapeutic alternatives and current approach for ovarian stimulation in patients with PCOS.
