Özet

Amaç Acil servislere müracaat eden geriatrik hasta sayısı artmaktadır. Bu mü- racaatların önemli bir kısmının da ambulans ile yapıldığı bilinmektedir. Bu çalışmada acil servise ambulans ile müracaat eden geriatrik olgulara ait verilerin analizini yapmak amaçlanmıştır. Gereç ve Yöntem Bu çalışma tanımlayıcı tipte bir araştırmadır. Acil servise başvuran geri- atrik olgulara ait ambulans nakil formları, hasta dosyaları, hastane bilgi sisteminde yer alan veriler geriye dönük olarak toplandı. Ambulans ile başvuran geriatrik hasta grubuna ait acil serviste kalış süresi, taburcu- luk ve hastane yatış oranı verileri, ayaktan başvuran geriatrik hasta gru- bu ile karşılaştırıldı. Bulgular Bir yıllık sürede acil servise başvuran hasta sayısı 93.609 kişi idi. Bu has- talardan %19.02’si (n=17.812) geriatrik (≥65 yaş) hastadır. Geriatrik has- taların %5.58’i (n=994) acil servise ambulans ile müracaat etmiştir. Am- bulans ile acil servise müracaat eden geriatrik hastalar, tüm ambulans ile müracaatların %37.45’ini oluşturmaktadır. Ambulans ile başvuran geriatrik hastaların %27.26’sı (n=271) hastaneye yatırılmıştır. Ambulans ile müracaat eden geriatrik hastaların acil serviste kalış süreleri ayaktan başvuran grup ile karşılaştırıldığında istatistiksel olarak anlamlı düzey- de düşük bulundu (p≤0.001). Sonuç Ambulans ile acil servise başvuran geriatrik hastalar ayaktan başvuru yapanlara göre istatistiksel olarak anlamlı düzeyde yüksektir. Ambulans ile müracaat eden olguların acil serviste kalış süreleri ayaktan müracaat edenlere göre daha az, hastane yatış oranları ise daha fazladır. Nefes darlığı, geriatrik hasta yaş grubunda hem ambulans hem de ayaktan yapılan başvurularda en sık yakınmadır.

Abstract

Objectives The volume of elderly patients presenting to emergency departments (ED) has been increasing. Many times elderly patients are conveyed to the ED via the ambulance. In this study, we aimed to analyze how many elderly patients presented to the ED via the ambulance. Methods In this descriptive and retrospective study, collected data of elderly pa- tients presenting to emergency department encompasses patient report forms of ambulance, charts, data belongs to patient in the hospital data system. These data were compared to data of elderly patients admitted by themselves including length of stay, discharge status and rate of hos- pitalization. Results Over a one-year period 93,609 patients were admitted to the emergency department and 19.02% of them were considered elderly or over the age of 66.0 years. Out of the elderly patients that presented to the ED, 5.58% of were transported by ambulance. For all ambulance runs made to the ED, 37.45% of them conveyed elderly patients, and 27.26% of these pa- tients were hospitalized. Length of hospital stay was significantly longer for elderly patients that were transported via the ambulance versus el- derly patients that transported themselves to the ED (p≤0.001). Conclusions Shortness of breath was the most common complaint observed for all el- derly patients that presented to the ED. The duration of ED length of stay for elderly patients transported by the ambulance was shorter than that of their counterparts that were not taken by ambulance. However, the length of hospital stay for elderly patients that were conveyed to the ED by ambulance was longer than that for elderly patients that transported themselves to the hospital.
