Erken Dönem Parkinson Hastalarında Pilates ve Elastik Bantlama Uygulamalarının Denge ve Postüral Kontrol Üzerine Etkisi: Randomize Kontrollü Pilot Çalışma

Özet

Amaç: Bu çalışmanın amacı erken dönem Parkinson hastalarında pilates ve elastik bantlama uygulamalarının denge ve postüral kontrol üzerine etkilerini karşılaştırmaktır. Yöntem: Bireyler randomize olarak pilates, elastik bantlama ve bekleme listesi kontrol grubu olmak üzere 3 gruba ayrıldı. Bireylere 6 hafta boyunca haftada 2 kez 60 dakikalık pilates eğitimi verildi. Elastik Bantlama Grubuna Pilates eğitimine ek olarak haftada 2 kez sırt bölgesine postüral düzeltme amacıyla elastik bant uygulandı. Bireylere eğitim öncesi ve sonrası denge ve postüral kontrol değerlendirmesi yapıldı. Berg Denge Ölçeği ve Gövde Bozukluk Ölçeği ile klinik denge değerlendirmesi; NeuroCom Balance Master denge ve performans test cihazı ile Kararlılık Sınırları, Otur Kalk, Normal ve Tandem Yürüme Testleri yapıldı. Bulgular: Pilates grubunda 6 erkek, elastik bantlama grubunda 2 kadın, 6 erkek ve Bekleme Listesi kontrol grubunda 3 kadın, 3 erkek olmak üzere toplam 20 bireyin verileri analiz edildi. Pilates ve elastik bantlama gruplarında Kararlılık sınırları testi reaksiyon süresinin 6 hafta sonra anlamlı olarak azaldığı, Normal yürüme testinde yürüme hızının ise anlamlı olarak arttığı görüldü. Topuk parmak ucu yürüyüş sonu salınımının elastik bantlama grubunda anlamlı olarak azaldığı tespit edildi. Elastik bantlama grubunda, Otur Kalk testinin vücut ağırlığı yükselme indeksinde 6 hafta sonra anlamlı artış görüldü (Wilcoxon Testi, p<0,05). Sonuç: Pilatesin Parkinson hastaları için uygulanabilir bir rehabilitasyon stratejisi olduğu, bu hastalarda dinamik denge ve dinamik postüral kontrolü iyileştirici etkisi olduğunu önermekteyiz. Elastik bantlamanın da düzgün postürü desteklemek amacıyla uygulanabilir bir yöntem olduğunu önermekteyiz.

Abstract

Introduction: The purpose of this study is to investigate the effects of Pilates training and elastic taping on balance and postural control in patients with early stage Parkinson’s disease (PD). Method: Patients were randomly divided into Pilates, elastic taping, and control (wait list) groups. Pilates training was performed for 6 weeks, twice a week and 60 minutes per session. In elastic taping group, in addition to Pilates training, elastic taping was applied to the upper back twice a week for 6 weeks with the aim of postural correction. In order to evaluate the postural control before and after the training, Berg Balance Scale and Trunk Impairment Scale were implemented and Limits of stability (LOS), Sit-to-Stand, Walk Across and Tandem Walking Tests were also performed by NeuroCom Balance Master performance test device. Results: The data of 20 patients (6 male in Pilates group, 2 female and 6 male in elastic taping group, 3 female and 3 male in control group) were analyzed. Reaction times in LOS test decreased significantly and walking speed in Walk Across test increased significantly after 6 weeks in Pilates and elastic taping groups. In elastic taping group, the postural sways of Tandem Walk test were decreased significantly and, the rising index in Sitto-Stand test increased significantly after 6 weeks (Wilcoxon Test, p<0.05). Conclusion: We suggest that Pilates is a feasible rehabilitation strategy for PD, and it has a remedial effect on the dynamic balance and postural control for these patients. We also suggest that elastic taping could be applied for supporting the right posture.
