Özet

Diastematomiyeli, spinal kordun uzunlamasına yarılmasıyla ortaya çıkan, fibröz veya kemik bir septum ile ayrılabilen iki hemikord oluşumuna neden olan, nadir bir okült spinal disrafizm’dir. Diastematomiyeli izole olabileceği gibi, miyelomeningosel, meningosel, spinal lipom, dermal sinüs, vertebral cisimlerin diğer segmental anomalileri veya visseral malformasyonlar gibi diğer disrafizmlerle ilişkili olabilir. Bu malformasyonun izole formu, iyi prognoza sahiptir. Bu nedenle, diastematomiyeli saptandığında doğru ebeveyn danışmanlığı sağlamak için, diğer anomalileri dışlamak adına prenatal değerlendirme dikkatle yapılmalıdır. Dikkatli sonografik inceleme ile diastematomiyeli’nin prenatal tanısı mümkündür. Spinal kanalda ekstra ekojenik posterior odak saptanması, bu durum için oldukça spesifik bir prenatal işarettir. Mağnetik rezonans görüntüleme (MRG), diğer ilişkili malformasyonları dışlamak için faydalıdır. İzole diastematomiyeli, çok az veya ilişkisiz defisitle ortaya çıkabilir. Bu olgu sunumunda biz, rutin ikinci trimester sonografisinde tespitedilen izole bir diastematomiyeli olgusunu sunduk.

Abstract

Diastematomyelia is a rare form of occult spinal dysraphism that is caused by longitudinal clefting of the spinal cord, which results in the formation of two hemicords that may or may not be separated by a fibrous or bony spur. Diastematomyelia may be isolated or associated with other dysraphisms such as myelomeningocele, meningocele, spinal lipoma, dermal sinus, other segmental anomalies of the vertebral bodies or visceral malformations. The isolated form of this malformation has a favorable prognosis. Thus to provide accurate parental counseling, prenatal evaluation should be done carefully to exclude other anomalies. Prenatal diagnosis of diastematomyelia is possible with careful sonographic examination. Detection of an extra echogenic posterior focus in the spinal canal is a highly specific prenatal sign for this condition. Magnetic resonance imaging (MRI) is beneficial to exclude other associated malformations. Here we present a case of diastematomyelia detected at routine second trimester sonography.
