Özet
Amaç Teratojenite Bilgi Servisleri (TBS), gebelik sırasında ilaç maruziyetine bağlı riski değerlendiren birimlerdir. Ancak ülkemizde henüz az sayıda olmaları nedeniy le bu değerlendirme çoğunlukla kadın hastalıkları ve doğum uzmanı hekimler tarafından yapılmaktadır. Bu çalışma, Göller Bölgesi (Burdur, Isparta) ve Antal ya’da çalışan kadın hastalıkları ve doğum uzmanla rının teratojenite danışmanlığı konusundaki tercih ve tecrübelerini değerlendirmeyi amaçlamıştır Gereç ve Yöntem Göller Bölgesi (Burdur, Isparta) ve Antalya’da çalışan kadın hastalıkları ve doğum uzmanlarına yüz-yüze görüşme tekniği kullanılarak anket uygulanmıştır. Bulgular Ankete katılan 103 hekimden soruların tamamını ce vaplayan 97 katılımcının anket formları değerlendi rilmiştir. Katılımcıların %64’ü kadın olan tüm grubun ortalama yaş ve mesleki deneyimleri sırasıyla 41,1 ± 8,7 ve 14,5 ± 8,5 yıl olarak saptanmıştır. Katılımcıların büyük çoğunluğu kamuya ait üçüncü basamak sağlık kuruluşunda çalışıyorken (%36,1), kamuya ait ikinci basamakta, özel hastanede ve kendine ait özel polik linikte çalışanların oranları sırasıyla %29,9, %30,9 ve %3,1 olarak belirlenmiştir. Hekimlerin %53,6’sı günde 20 ve üzeri gebe muayene ettiğini belirtirken, terato jenite risk danışmanlığı yapma sıklıkları %36,1’inde ayda en az 1, %32’inde her hafta ve %26,8’inde her gün şeklinde ifade edilmiştir. Teratojenite danışman lığı sırasında kullanılan kaynaklar internet (%77,3), kitaplar (%70,1), meslektaşlar (%34) ve olgu sunum ları (%16,5) olarak sıralanmıştır. Hekimlerin %87,4’ü bu kaynakları yılda birkaç kereden fazla, geri kalanı da yılda birkaç kereden az kullandıklarını belirtmiş tir. En sık danışmanlık verilen ilaç grupları antibiyo tikler (%69,1), analjezikler (%45,4), antidepresanlar (%26,8) ve antiepileptikler (%17,5) olarak ifade edil miştir. Son 5 yılda teratojenite danışmanlığı ile ilgili eğitim alan hekim sayısı kısıtlıdır (%9,3). Hekimlerin %79,4’u ileri danışmanlık için hastalarını bir perinata loji uzmanına yönlendirirken, tıbbi farmakoloji uzma nı veya TBS’ye yönlendirme oranı %14,4 düzeyinde kalmıştır. Sonuç Teratojenite danışmanlığı; gebelik ve kullanılan ilaca ait detaylı öyküye hakim olmayı, güncel ve detaylı bir literatür taramasını ve hastanın ilaca bağlı risk artışı nı tam olarak anladığından emin olmayı gerektiren bir süreçtir. Devam eden gebeliklerin takibi ise, ilaca iliş kin önemli epidemiyolojik bilgiler sağlayabilir. Mevcut anket verilerinin nitel yöntemlerle detaylandırılması konuyla ilgili daha kapsamlı bilgi sağlayacaktır.
Abstract
Objective Teratogenicity Information Services (TIS) are the units that assess the risk associated with drug expo sure during pregnancy. However, since they are not yet common in our country, this evaluation is mostly made by obstetrics and gynaecology specialists. This study aimed to evaluate the preferences and expe riences of gynecologists and obstetricians in the La kes Region (Burdur, Isparta) and Antalya on teratoge nicity counseling. Materials and Methods A questionnaire was applied to gynecologists and obstetricians working in Lakes Region (Burdur, Ispar ta) and Antalya via a face-to-face interview technique Results Questionnaires of 97 participants who answered all of the questions from 103 physicians that participated the survey were evaluated. The mean age and profes sional experience of the whole group, of which 64% of the participants were women, were determined as 41.1 ± 8.7 and 14.5 ± 8.5 years, respectively. While the vast majority of the participants work in a public tertiary healthcare facility (36.1%), the rates of tho se working in public secondary care, private hospitals and their own private polyclinics are 29.9%, 30.9%, and 3.1%, respectively. While 53.6% of the physi cians stated that they examine 20 or more pregnant women per day, the frequency of teratogenicity risk counseling was at least one a month in 36.1%, every week in 32% and every day in 26.8%. The sources used for teratogenicity counseling were listed as in ternet (77.3%), books (70.1%), colleagues (34%), and case reports (16.5%). 87.4% of the physicians stated that they use these resources more than a few times a year and the rest less than a few times a year. The most frequently counseled drug groups were antibi otics (69.1%), analgesics (45.4%), antidepressants (26.8%), and antiepileptics (17.5%). The number of physicians who received training on teratogenicity counseling in the last 5 years is limited (9.3%). While 79.4% of the physicians referred their patients to a perinatology specialist for further counseling, the rate of referral to medical pharmacology specialists or TIS remained at 14.4%. Conclusion Teratogenicity counseling is a process that requires a detailed history of pregnancy and the drug used, an up-to-date and detailed literature review, and making sure that the patient fully understands the drug-rela ted risk increase. The follow-up of ongoing pregnan cies can provide important epidemiological informa tion about the drug. Elaborating the current survey data with qualitative methods will provide more comp rehensive information on the subject.
