THE EFFECT OF RADIOTHERAPY ON SURVIVAL IN NASAL CAVITY AND PARANASAL SINUS TUMORS

Özet

AmaçNazal kavite ve paranazal bölge tümörleri nadir görülenve genellikle ileri evrede tanı konulan tümörlerdir.Bu çalışmanın amacı sinonazal bölge tümörlerindehistopatolojik tanının, boyun lenf nodu tutulumunun vetedavi modalitelerinin sağkalıma etkisini araştırmaktır.Gereç ve Yöntem1991- 2002 yılları arasında kliniğimize paranazal sinüskanseri tanısıyla başvuran ve tedaviye alınan 47hastadan 37’sinin verileri retrospektif olarak değerlendirildi.Hastaların 16’sına (%43.24) cerrahi + postoperatifradyoterapi (PORT), 6’sına (%16.21) cerrahi+ PORT + kemoterapi (KT), 11’ine (%29.72) definitifradyoterapi (RT), 4 hastaya (%10.81) RT ardındanKT uygulanmıştır. Sağkalım analizleri Kaplan-Meierve log rank testleri ile yapılmıştır.BulgularTakip süremiz 22 aydır. Hastaların 24’ü erkek, 13’ükadındır. Medyan yaş 55’tir. Hastaların histopatolojiktanılarının çoğunluğunu sırasıyla yassı hücreli kanser,adenoidkistik karsinom ve baş – boyun sarkomu oluşturmaktadır.T3-T4 hasta sayısı 23 (%76.6)’dü. Beşhastanın boyun lenf nodu tutulmuştur. Çalışmamızdaboyun lenf nodu tutulumunun sağkalımı olumsuzetkilediğini (p=0.03), adenoidkistik karsinomun yassıhücreli kanser ve baş boyun sarkomuna göre iyi prognostikolduğu gösterilmiştir (sırasıyla p=0.01, p=0.03).Definitif RT ile PORT tedavi sonuçlarımız karşılaştırıldığındaistatistiksel anlam içermese de (p=0.17),PORT’ nin sağkalıma etkili olduğunu düşündüreceksonuçlara varıldı. 5 yıllık sağkalım oranı % 43.69 olarakbulunmuştur.SonuçÇalışmamızda hasta sayısı kısıtlı olmasına rağmensağkalım sonuçlarımız literatür ile uyumlu bulunmuştur.Sağkalımı etkileyen faktörlerin değerlendirilmesiiçin daha geniş hasta sayılı çalışmalara ihtiyaç vardır.

Abstract

Objective Nasal cavity and paranasal tumors are rare and usually diagnosed in advanced stage. This study was retrospectively designed to investigate the survival effect of histopathological diagnosis, lymph node involvement and treatment modalities in sinonasal tumors. Material and Methods Thirty seven patients out of 47 who had referred to our clinic with paranasal sinus cancer between 1991-2002 were included in the study. Twenty-four patients were male and thirteen were female. Median age was 55 years. Most seen histopathological diagnoses were squamous cell cancer, adenoidcystic carcinoma and head and neck sarcoma, respectively. The number of patients with T3-T4 was 23 (76.6%). There was lymph nodes involvement in five patients. Surgical treatment + postoperative radiotherapy (PORT) was performed in 16 patients (43.24%), surgical treatment + chemotherapy (CT) was performed in 6 patients (16.21%), and definitive radiotherapy (RT), was performed in 4 patients (10.81%). Results The follow-up period was 22 months. Five-year survival rate was 43.69%. In our study, we showed that neck lymph node involvement negatively affects survival (p=0.03), adenoidcystic carcinoma increases survival compared to squamous cell cancer and head and neck sarcoma (p=0.01, p=0.03, respectively). Although the results of definitive RT and PORT treatment were not statistically different (p = 0.17), it was concluded that PORT was effective in survival. The 5-year survival rate was 43.69%. Conclusion Although the number of patients in our study was limited, our survival results were found to be consistent with the literature. Studies with more numbers of patients are needed to make factors effecting survival clear.
