Özet

AmaçÇalışmanın amacı, peroperatif farklı dozlarda uygulananlidokain infüzyonunun postoperatif analjezi ihtiyacınaetkilerini, postoperatif bulantıya ve toplam parasetamoltüketimine etkilerini araştırmak.Gereç ve YöntemBu çalışmada operasyon süresi 120 dk’ı geçmeyenelektif şartlarda TAH-BSO operasyonu geçiren ve peroperatiflidokain infüzyonu yapılan hastaların medikalve anestezi kayıtları retrospektif olarak değerlendirildi.18-65 yaş arası, ASA I-II, VKİ <25 olan hastalar çalışmayadahil edildi. Dosya kayıtları incelenen hastalar3 gruba ayrıldı; Grup L1: 1.5 mg/kg lidokain infüzyonyapılan hastalar, Grup L2: 3 mg/kg lidokain infüzyonyapılan hastalar ve Grup S: sadece SF infüzyon yapılanhastalar. Değerlendirilen dosyalardan 3 mg/kg/saat lidokain infüzyonu yapılan 3 hastada bradikardigeliştiği için lidokain infüzyonunun kesildiği saptandıve çalışma dışı bırakıldı. Hastaların anestezi takipfişlerinden hemodinamik parametreler, demografikveriler, algoloji takip formlarından postoperatif vizualanalog skala skorları, bulantı, ilk mobilizasyon zamanıve toplam parasetamol tüketimi kaydedildi.BulgularDemografik ve hemodinamik veriler açısından gruplararasında fark bulunmadı. Postoperatif 2, 6 ve 14.saat VAS değerleri Grup L1’de Grup S’e göre anlamlıolarak daha düşük bulundu (p<0,001). Hastalarınpostoperatif 6, 10 ve 14. saatteki bulantısı Grup S’dediğer gruplara göre anlamlı olarak daha yüksek bulundu(p<0.05). İlk analjezi zamanlarına bakıldığındada Grup L1’de Grup S’e göre ilk parasetamol zamanıanlamlı olarak daha geç bulundu (p=0.008).SonuçSonuç olarak 3 mg/kg lidokain infüzyonun postopeartifvizual analog skala skorlarını düşürdüğünü ancakbradikardi yapma olasılığının olduğunu, 1.5 mg/kg lidokain infüzyon dozlarının postoperatif analjeziketki sağladığını ve yan etki insidansını da azalttığınıdüşünmekteyiz.

Abstract

Objective The aim of this study is to investigate the effects of different doses of lidocaine infusion on postoperative analgesia requirement, postoperative nausea and total paracetamol consumption. Materials and Methods In this study, medical and anesthesia records of patients who underwent TAH-BSO that operation time not exceeding 120 minutes, under perioperative lidocaine infusion were evaluated retrospectively. Patients aged between 18-65 years with ASA I-II and BMI <25 were included in the study. The patients whose records were investigated were divided into 3groups. Group L1: 1.5 mg / kg lidocaine infusion, Group L2: 3 mg / kg lidocaine infusion and Group S: patients receiving only physiological saline solution infusion. Due to the bradycardia in 3 patients who received 3 mg/ kg/h lidocaine infusion, were excluded from the study. Hemodynamic parameters, demographic datas, postoperative visual analogue scale scores, nausea, first mobilization time and total paracetamol consumption were recorded from anesthesia and algology patient charts. Results No difference was detected between the groups in terms of demographic and hemodynamic data. Postoperative at 2nd,6th and 14th hours VAS scores were significantly lower in Group L1 than Group S (p <0.001). Nausea at the postoperative 6th,10th and 14th hours were significantly higher in Group S than in the other groups (p <0.05). When the first analgesia times were investigated, the first paracetamol time was significantly later in Group L1 than Group S (p = 0.008). Conclusions In conclusion, we believe that 3 mg /kg lidocaine infusion decreases postoperative visual analogue scale scores despite the possibility of bradycardia, and 1.5 mg/kg lidocaine infusion doses provide postoperative analgesia and decrease the incidence of side effects.
