Özet

Kanserli hastalarda kullanım sıklığı giderek artan santral venöz kateterlerin uygulaması ve komplikasyonlarına ilişkin sonuçlarımızı sunmak amaçlanmıştır. Solid tümörleri nedeniyle infüzyonel kemoterapi uygulanan 112 hastaya toplam 114 santral venöz kateter (SVK) (eksternal ve implantabl kateter) takıldı. Hastalar yaş, performans durumu, tümör tipi, evresi, komorbidite, aldıkları tedavi türü, kateter tipi, takılma şekli, süresi ve komplikasyonları (erken ve geç) bakımından değerlendirildi. Çoğunluğunu (82 hasta, %73.2) kolorektal tümörlü hastaların oluşturduğu 112 solid tümörlü hastaya 101 (%88.6) eksternal ve 13 (%11.4) implantabl olmak üzere toplam 114 SVK takıldı. Median kateter taşıma süresi 157.7±64.6 gün, toplam kateter izleme süresi 18342 gün idi. Kateterlerden 15 (%13.1)'inde komplikasyon gelişti. Bunların 13'ü eksternal SVK olup, 6 (%5.2)'sı erken, 9 (%7.9)'u da geç komplikasyon şeklindeydi. Hem erken hem de geç komplikasyonları n içinde en sık izleneni malfonksiyon idi. Total komplikasyon hızı 0.076/100 kateter günü idi. Solid tümörlü hastalarda SVK düşük komplikasyon oranına sahip, tedavi kolaylığı sağlayan venöz giriş sistemleridir.

Abstract

To evaluate the efficacy and complications of central venous access systems in patients with solid tumors. External and implantable central venous catheters were implanted in patients who would receive chemotherapy for cancer. All data such as the patient's age, performance status, tumor type, stage, comorbidity, type of chemotherapy and catheter, catheter duration and early and late complications were collected. Totally, 114 central venous catheters were implanted to 112 patients who had various solid tumors. Most of the patients (82/112, 73.2%) had colorectal cancer. Totally 101 (88.6%) external and 13 (11.4%) implantable central venous catheters were used. The median follow-up was 157.7±64.6 days with a total of 18342 days. Complications were observed in 15 (13.1%) of 114 catheters, and 13 of them were external catheters. Six and 9 complications were early and late complications respectively. The most frequent complication observed in whole group was malfunction. The total complication rate was 0.076/100 days. The central venous catheters are the venous access systems which facilitate the treatment with a rate of low complication in patients with solid tumors.
