Non-Hodgkin Lenfomalı Hastalarda Hepatit B Prevalansının Değerlendirilmesi ve Profilaksinin Yeri

Özet
Amaç: Hepatit B virüs (HBV), kemoterapi alan hastalarda morbidite ve mortalitenin önemli bir nedenidir. Bu çalışmada amacımız tedavi almış olan Non-Hodgkin lenfoma (NHL) hastalarında hepatit B serolojisi, hepatit B aktivasyonu ve proflaksi alma durumlarını araştırmaktır. Hastalar ve Yöntem: 2013-2020 yılları arasında tedavi edilen NHL hastalarının dosyaları retrospektif olarak incelenerek HBsAg, Anti-HBs, Anti-HBc IgG parametreleri ve demografik bilgileri kaydedildi. Bulgular: 147 hastasının 117’sinde Anti-HBc IgG bakılmış olup 52 (%45)’si reaktif, 65 (%55)’i nonreaktif idi. HbsAg bakılan 136 hastanın 2 (%1.5)’si reaktif 134 (%98.5)’ü nonreaktifti. 127 hastada Anti-HBs bakılmış olup, antikor bakılanlardan 20 tanesi (%16) reaktif, 107 tanesi (%84) nonreaktifti. Anti-HBc IgG reaktif 52 hasta ve HBsAg reaktif iki hastaya antiviral tedavi başlanmış ancak iki hasta antiviral tedaviye devam etmeyerek mortal seyretmiştir. Sonuç: Anti-HBc IgG ya da HBsAg pozitif olan ve immünsüpresif tedavi alacak tüm hastalar reaktivasyon riski açısından değerlendirilmeli ve risk faktörlerine göre profilaksi planlanmalıdır. Abstract
Objective: Hepatitis B virüs (HBV) is one of the important reasons of the mortality and morbidity in patients receiving chemotherapy. We aimed to investigate the serology of HBV, prophylaxis status and HBV reactivation status in patients with Non-Hodgkin lymphoma (NHL) who received therapy. Patients and Methods: The data of the patients with NHL who received chemotherapy between 2013-2020 were retrospectively evaluated and demographic characteristics, HBsAg, Anti-HBs and Anti-HBc IgG levels were recorded. Results: 117 of 147 patients were checked for Anti-HBc IgG and 52 patients (45%) were reactive, 65 pateints were nonreactive (55%). HBsAg were tested in 136 patients and 2 of them were reacitve (%1.5) and 134 of them (%98.5) were nonreactive. One hundred twenty sevenpatients were checked for Anti-HBs and 20 of them (16%) were reactive and 107 of them (84%) were nonreactive. Fifty two patients have reactive Anti-HBc IgG and two patients have reactive HBsAg results and they received antiviral therapy, neverthless 2 of them were not used the antiviral therapy regularly, so they died. Conclusion: All patients, who were expected to receive immunosuppressive therapy and had reactive Anti-HBc IgG or HBsAg, should be evaluated for the risk of HBV reactivation and according to the risk factors, eligible patients should receive prophylactic antiviral therapy.
