Over kistlerinde kist duvarının eksizyonu fonksiyonel over dokusunun kaybına yol açıyor mu?

Özet
Amaç: Bu çalışmada kist duvarı eksizyonunun, doku koruyucu bir teknik olup olmadığının değerlendirilmesi amaç¬landı. Kurum. Mersin Üniversitesi Tıp Fakültesi, Kadın Hastalıkları ve Doğum AD. ve Patoloji AD Mersin Materyal ve Metod: Ultrasonografıde saptanan persistan over kisti, kronik pelvik ağrı veya infertiİite nedeni ile başvuran ve unilateral over kisti tanısı alarak cerrahi tedavi uygulanan 60 olgu bu çalışma kapsamına alındı. Olgulara ait demografik ve klinik veriler toplandı. 31 olgu postoperatif endometriotik kist, 11 olgu dermoid kist, 11 olgu basit fonksiyonel kist, 5 olgu seröz kistadenom ve 2.olgu müsinöz kistadenom tanısı aldı. Histopatolojik değerlendirme ve gradeleme semikantitatif skala kullanılarak, eksize edilen kist duvarinü komşu over dokusunun olup olmamasına göre yapıldı Sonuçlar: Eksize edilen 60 kist duvarından 43' ünde (% 71,6) değişen derecelerde over dokusu saptandı. Endometriotik kist tanısı alan 27 olguda (% 87) ve non endometriotik kist olarak değerlendirilen 16 olguda (% 55) over dokusu saptandı. Eksize edilen kist duvarındaki ovaryen dokunun miktarıyla histopatolojik tanı arasında ilişki izlenmedi (p=0,064). Preoperatifultrasonografi ile ölçülen kist boyutu ile kist duvarındaki rezidüel over dokusu arasında istatistiksel olarak anlamlı bir ilişki mevcuttu (p—0,02). Tartışma: Eksize edilen endometriotik kistlerin % 87' sinde psödökistle beraber değişen derecelerde over dokusu izlenmiştir. Öte yandan endometriotik kistlerin sadece ikisinde (% 7,86) normal over dokusuna benzer nitelikte over dokusu izlenmiştir. Kist eksizyonu fonksiyonel over dokusunda kayba yolaçmamaktadır.
Abstract
SUMMARY: Objective: In this study we evaluated that whether the stripping technique (or excision of cyst wall) causes loss of functional ovarian tissue. Materials and Methods: Sixty women who underwent surgery due to scan detected mass, chronic pelvic pain and infertility and acute abdominal pain were enrolled in this study. Demographic and clinical datas were recorded. Thirty-one cases were evaluated as endometriotic cyst, 11 cases were diagnosed as dermoid cyst, 11 cases were evaluated as simple functional ovarian cyst, 5 cases were evaluated as serous cystadenoma and ,2 cases were evaluated as mucinous cystadenoma. Histopathologic evaluation and grading according to semiquantitative scale of excised cyst wall were done to determine the presence of ovarian tissue adjacent the cyst wall. Results: Ovarian tissue adjacent to cyst wall was seen in 43 cases (71,6 %) of ovarian cysts. Twenty-seven cases of endometriotic cysts (87 %) and in 16 cases (55 %) of nonendometritic cysts had ovarian tissue tissue adjecent to excised cyst wall. There is no statistical difference between the endometriotic cysts and non-endometriotic cysts for presence of normal ovarian tissue adjacent to removed cyst wall (p=0,064). However there is a statistically significant difference between the preoperatively measured ultrasonographic diameter of ovarian cyst and presence of normal ovarian tissue adjacent to removed cyst wall (p=0,02). Conclusion: In endometriotic cysts 87 % of the specimens included some ovarian tissue excised along with the cyst pseudocapsule. Only 7, 86 % of excised endometriotic cysts were showed similar morphologic architecture as normal ovary. However cyst excision could not result with loss of functional ovarian tissue.
