Laparoscopic management and its outcomes in cases with nonpalpable testis

Özet

Objective: Diagnostic laparoscopy is the gold standard in the algorithm of nonpalpable testis. Testiculartissue is examined and treatment is planned accordingly. In this study we reviewed the place of diagnosticlaparoscopy, and evaluated the results and effectiveness of laparoscopy in the diagnosis and management ofnonpalpable testis.Material and methods: Children who had diagnostic laparoscopy for nonpalpable testes were included inthe study. Physical examination results, ultrasonography (USG) reports, age at surgery, laparoscopic and inguinalexploration findings, surgical procedures, orchiopexy results, early and late-term complications wereevaluated. Follow-up visits were performed at 3-month intervals for the first, at 6-month intervals for the2. year, then at yearly intervals. Testicular size and location was evaluated by during control examination.Results: Overall 58 boys, and 68 testes (26 left: 44.8%; 22 right: 37.9%, and 10 bilateral: 17.2%) were includedin the study. Mean age at surgery was 5.5 years (10 months-17 years). Diagnostic value of USG was 15.7%. Diagnosticlaparoscopy findings were as follows: Group 1: blind-ended vessels, n=7 (10.2%); Group 2: intraabdominaltestes, n=8 (11.7%); Group 3: vas and vessels entering internal ring, n=53 (77.9%). Overall 43 testes underwentorchiopexy, which were normal (n=8) or hypoplastic (n=35). Mean follow- up period was 19 months (1-12 years),and on an average 7 visits were performed (5-14). On follow-up, 5 testes were normal- sized and located in thescrotum, while 4 testes were atrophic and underwent orchiectomy. Two testes were found in the inguinal canaland redo orchiopexy was performed. Control USG revealed reduced testicular blood supply and volume.Conclusion: Laparoscopic surgery is safe and effective in the management of nonpalpable testes. In themajority, routine use of diagnostic laparoscopy in the algorithma does not confer any additional contributionsin many patients

Abstract

Amaç: Ele gelmeyen testis olgularının yönetiminde tanısal laparoskopi altın standart olup testis dokusu araştırılır ve buna göre tedavi planlanır. Bu çalışmada ele gelmeyen testis olgularında tanısal laparoskopinin algoritmadaki yerini gözden geçirip tanı ve tedavi sonuçlarımızı değerlendirdik. Gereç ve yöntemler: Ele gelmeyen testis olgularında tanısal laparoskopi uygulanan çocuk hastalar çalışmaya alındı. Hastaların muayene bulguları, ameliyat yaşı, ultrasonografi (USG) raporları, laparoskopik ve inguinal eksplorasyon bulguları, cerrahi işlemler, erken ve geç dönem komplikasyonlar ile orşiopeksi sonuçları değerlendirildi. Hasta takibi 1. yıl 3 ayda bir, 2. yıl 6 ayda bir ve sonrasında yıllık yapıldı. Kontrol muayenede testis lokalizasyonu ve boyutu değerlendirildi. Bulgular: Toplam 58 hasta ve 68 testis ünitesi; 26’sı sol (%44,8), 22’si sağ (%37,9), 10’u bilateral (%17,2) çalışmaya dahil edildi. Ameliyat yaşı ortalama 5 yıl 6 ay (10 ay-17 yıl) ve USG’nin tanısal değeri %15,7 bulundu. Tanısal laparoskopide grup 1’de kör sonlanan damarlar, 7 olgu (%10,2); Grup 2’de intraabdominal testis, 8 olgu (%11,7); Grup 3’de internal ringden giren kord ve damarlar, 53 olgu (%77,9) görüldü. Toplam 43 testise orşiopeksi yapıldı; bunların 8’i normal, 35’i hipoplazikti. Takip süresi 1 yıl 7 ay (1-12 yıl) ve takip sayısı 7 (5-14) olarak belirlendi. Kontrol muayenede sadece 5 testis normal lokalizasyon (skrotum) ve boyutta bulundu. 4 Testiste atrofi gelişti ve orşiektomi yapıldı; 2 testis inguinal kanalda bulundu ve redo orşiopeksi yapıldı. Kontrol USG’lerde testis kanlanması ve hacmi düşük bulundu. Sonuç: Ele gelmeyen testis olgularının yönetiminde laparoskopi güvenli ve etkilidir. Tanısal laparoskopinin algoritmada rutin kullanımı birçok hastada ek katkı sunmamaktadır
