A rare complication of ureteral double-J stenting after flexible ureteroscopy: renal parenchymal perforation

Özet

Üreteral double-j (DJ) stent yerleştirilmesi farklı endoürolojik girişimleri takiben oldukça yaygın olarak kullanılmaktadır. DJ stent uygulamasına ait çok sayıda komplikasyonlar ortaya konmuş olmasına rağmen, günümüze kadar sadece 3 böbrek parankim perforasyonu olgusu bildirilmiştir. Biz, 35 yaşındaki bir bayan hastamızda, üreter üst bölüm taşına yönelik fleksibl üreteroskopiyi (FURS) takiben yerleştirdiğimiz ürete- ral DJ stent komplikasyonu olarak saptadığımız, perirenal hematomun eşlik etmediği bir böbrek parankim perforasyonu olgusunu sunduk. Renal kolik ile acil servise başvuran hastanın tanısı kontrastsız bilgisayarlı tomografi (BT) ile kondu. Hasta postoperatif ikinci günde DJ stentin proksimal ucunun floroskopi eşliğinde toplayıcı sistem içerisine geri çekilmesi ile başarılı bir şekilde tedavi edildi.

Abstract

Double-j (DJ) ureteral stenting is a very common procedure performed after various endourological pro- cedures. Although several complications due to DJ stent insertion were revealed, only three cases of renal parenchymal perforation have been reported to date. We present a case of a 35- year-old woman who had perforation of renal parenchyma without perirenal hematoma following insertion of DJ ureteral stent after flexible ureteroscopy for the treatment of an upper ureteral calculi. Non-contrast computed tomography was used for diagnosis. Patient was successfully managed with repositioning of the stent under fluoroscopic guidance on postoperative second day postoperatively.
