Özet

Amaç: Zor havayolunun tahmini genel anestezi öncesindeki en önemli zorluklardan birisidir. Bu çalışmanın amacı son zamanlar- da geliştirilen bir skorlama sistemi olan FRONT skorun klinik yararlılığını değerlendirmektir. Yöntemler: Bu çok merkezli, inter-observer (gözlemciler arası), prospektif ve çift-kör çalışmaya iki üniversite hastanesindeki 976 hasta dahil edildi. Hastaların pre-operatif değerlendirmeleri ha- vayolu yönetiminde beklenen zorluğu değerlendiren ve skorlayan anestezi uzmanlarından oluşan bir ekip (A) tarafından gerçek- leştirildi. Bu A ekibinden bağımsız bir şekilde çalışan intraope- ratif hava yolunu değerlendiren diğer ekipte (B) gerçek havayolu enstrümantasyonunu yaptı ve gerçek bulguları puanladılar. Her iki takım da FRONT skorlama sistemini kullandılar ve kör de- ğerlendirme yapmak için birbirinden bağımsız çalıştılar. Pre- and intra-operatif FRONT skorlarının istatiksel analizi çevrimdışı kör olarak yapıldı. Bulgular: Çalışma sonuçlarımıza göre, pre-operatif karma FRONT skoru ile indüksiyon aşamasında gözlemlenen skor ara- sında kayda değer ve umut verici bir ilişki vardır (Spearman=0,43). Skor ögeleri arasında en iyi korelasyon F ve R ögeleri (sırasıyla kappa=0,44 ve 0,36) için ve en kötü korelasyon O ve T ögeleri (sırasıyla kappa=0,25 ve 0,24) için gözlenmiştir. Sonuç: Havayolu durumunun tahmini ve dokümantasyonu için kullanılan FRONT formülü havayolu yönetimi zorluklarını de- ğerlendirmek ve tanımlamak için basit ve etkili bir yöntemdir. Sistemin duyarlılığını ve özgüllüğünü değerlendirmek için başka prospektif çalışmalara gereksinim vardır

Abstract

Objective: The prediction of difficult airway is one of the most im- portant challenges before general anaesthesia. This study aimed to assess the clinical usefulness of the FRONT score, a recently devel- oped scoring system to predict and document airway difficulties. Methods: This multi-centre, inter-observer, prospective and dou- ble-blinded study included 976 patients from two university cen- tres. The pre-operative evaluation of the patients was performed by a pre-operative team of anaesthesiologists (team A) who eval- uated and scored the expected difficulty of airway management. An intra-operative team of evaluators (team B) working inde- pendently of team A, performed the actual instrumentation of the airway and scored the actual findings. Both teams used the FRONT scoring system and worked independently of each other to ensure blinded assessments. The statistical analysis of the pre- and intra-operative FRONT scores was performed in an off-line blinded manner. Results: Our results show a fair and promising association between pre-operative composite FRONT score and that observed at the induc- tion phase (Spearman=0.43). Among the score components, the best correlation was observed for the F and R components (kappa=0.44 and 0.36, respectively), and the worse correlation was observed for the O and T components (kappa=0.25 and 0.24, respectively). Conclusion: The FRONT formula for the prediction and doc- umentation of the airway status is a simple and effective method for assessing and defining airway management difficulties. Further prospective studies are required to assess the sensitivity and spec- ificity of the system.
