Effect of Two Techniques of Parental Interaction on Children’s Anxiety at Induction of General Anaesthesia-A Randomized Trial

Objective: Several non-pharmacological techniques, such as parentalpresence and behavioral preparation, are used to decreasechildren’s anxiety at anaesthesia induction. We compared themean anxiety score in children at the time of anaesthesia inductionwith two different physical techniques of parental interactionand a control group with no parent present. The secondaryobjective was to determine the face mask acceptance during induction.Methods: This study recruited 123 ASA I & II children, aged 1to 8 years, undergoing day care surgery, who were randomly allocatedto three groups. Children either went to the operating room(OR) alone (Gp C), or one parent sat next to the child at induction(Gp PS), or the child sat in parent’s lap (Gp PH). The anxietyscore on the modified Yale Preoperative Anxiety Scale (mYPAS)was recorded in the preinduction area of OR and at the inductionof anaesthesia before the face mask application. A cut-off value ofless than 30 indicated low anxiety. The face mask acceptance wasalso rated.Results: All patients had the mYPAS scores higher than 30 in thepreinduction area with no significant difference between groups.Prior to induction, the Gp C score was significantly high as comparedto Gp PS (p=0.016) and Gp PH (p=0.001), but it was notdifferent between the Gp PS and PH (p=1.00). The face maskacceptance was easy in 4.9 % patients in Gp C, 26.8% in Gp PS,and 56% in Gp PH.Conclusion: Parental presence during induction did not preventchildren’s anxiety, but it reduced it, irrespective of thephysical technique used. The face mask acceptance was betterin Gp PH.
Abstract
Amaç: Anestezi indüksiyonunda çocukların anksiyetesini azaltmak için, ebeveynlerin bulunması ve davranışsal hazırlama gibi bazı farmakolojik olmayan teknikler kullanılmaktadır. Bu çalışmada, anestezi indüksiyonunda çocuklarda ortalama anksiyete skorunu, parental etkileşimin olduğu iki farklı fiziksel tekniğin kullanıldığı gruplarda ve ebeveynlerin bulunmadığı kontrol grubunda karşılaştırdık. Sekonder amacımız indüksiyon boyunca yüz maskesi kabul oranını belirlemekti. Yöntemler: Çalışmaya yaşları 1 ile 8 arasında değişen, günübirlik cerrahi uygulanan, ve rasgele üç gruba ayrılan ASA 1 ve II skorlu 123 çocuk dahil edildi. Çocukların bir grubu ameliyathaneye yalnız gittiler (Gp C), bir grupta bir ebeveyn indüksiyon sırasında çocuğun yanında oturdu (Gp PS), ve diğer grupta çocuk ebeveyninin kucağında oturdu (Gp PH). Anksiyete skoru Modifiye Yale Preoperatif Anksiyete Skalası (mYPAS) kullanılarak, ameliyathanenin pre-indüksiyon bölümünde ve yüz maskesi uygulamasından önce anestezi indüksiyonunda kaydedildi. 30’un altındaki cut-off değeri düşük anksiyeteyi gösterdi. Yüz maskesi kabul oranı Gp C’de %4,9, Gp PS’de %26,8 ve Gp PH’de %56 olarak saptandı. Sonuç: İndüksiyon boyunca ebeveynlerin bulunması, hangi fiziksel teknik kullanılırsa kullanılsın, çocuklarda anksiyeteyi tamamen önlemedi, ancak azalttı. Yüz maskesi kabul oranı PH grubunda (Gp PH) daha iyiydi.
