KOAH akut atağında Chlamydia pneumoniae, Mycoplasma pneumoniae, Legionella spp. ve Influenza A sıklığı

Özet

KOAH akut atağında infeksiyon önemli bir yer tutmaktadır. Akut ataktan sorumlu etkenler arasında en sık Haemophilus influenzae, Streptococcus pneumoniae ve Moraxella catarrhalis yer almaktadır. Son yıllarda, başta C.pneumoniae olmak üzere atipik bakteriler KOAH akut atağında çalışılmıştır. Bu çalışmada Chlamydia pneumoniae, Mycoplasma pneumoniae, Legionella pneumophila ve Influenza A antikorlarını KOAH akut atağıyla gelen olguların serumlarında ELISA yöntemiyle araştırdık. Çalışmaya KOAH akut atağı geçiren 39 olgu ve kontrol grubu olarak 20 sağlıklı kişi alınmıştır. KOAH'lı olguların yaş ortalamaları 60.4±9.7 (14K, 25E) ve kontrol grubunun yaş ortalaması 55±8.4 (11K, 9E) idi. Olguların ortalama FVC değeri 2.21±1.19 L, FEV1 1.37±0.9 L, FEV1/FVC ise % 60.9±10.4 idi. Ortalama hastanede yatış süreleri, seropozitif olanlarda 12.2±3.7 gün, seronegatif olanlarda 11.2±6.2 idi (p>0.05). Serolojik olarak Influenza A %17.8 (n:7), C. pneumoniae IgM %12.8 (n:5), C. pneumoniae IgA %7.7 (n:3), C. pneumoniae IgG %15.3 (n:6), M. pneumoniae %7.7 (n:3) ve Legionella spp. %2.7 (n:1) oranında pozitif bulunmuştur. Kontrol grubunda ise sadece bir kişide C. pneumoniae IgG pozitif idi. Bu sonuçlara göre, KOAH akut atağının en sık nedeni olarak influenza A ve C.pneumoniae bulunmuştur. Sonuç olarak, Influenza A ve C. pneumoniae'nin de KOAH akut atağı geçiren olguların etiyolojisinde düşünülmesi gerektiği kanısındayız.

Abstract

Bacterial infections are frequent causes of COPD exacerbations. Haemophilus influenzae, Streptococcus pneumoniae and Moraxella catarrhalis are the major pathogens. Recently, atypical bacteria, especially Chlamydia pneumonia were studied in acute COPD exacerbation. In our study, the prevalance of C. pneumonia (IgA, IgM, and IgG), Mycoplasma pneumonia, Legionella pneumophila and Influenza A was assessed by serology in 39 patients with COPD. All antibodies were detected with ELISA. The average age of patients was 60.4±9.7 (M:25, F:14), and 55±8.4 of control group. (M:11, F:9). Mean FVC, FEV1 and FEV1/FVC values of the patients were 2.21±1.19 L, 1.37±0.9 L, and 60.9±10.4%, respectively. The duration of hospitalization of the patients was found as 12.2±3.7 days for seropositive cases, and 11.2±6.2 for seronegative cases (p>0.05). Influenza A was detected serologically in seven patients (17.8%), C. pneumoniae IgA, IgM and IgG antibodies were detected in 5 (12.8%), 3 (7.7%), and 6 (15.3%) patients with COPD, respectively. M. pneumoniae and Legionella pneumophila were detected in 3 patients (7.7%) and in one patient (2.7%) with COPD, respectively. C. pneumoniae IgG was positive only in one subject of control group. Our data show that Influenza A and C. pneumoniaes were frequent causes of COPD exacerbation. As a result, we believe that Influenza A and C. pneumoniae could be considered as a cause of COPD exacerbation.
