Özet
Son yıllarda, yoğun bakım ünitelerindeki gelişmelere rağmen Erişkinin Sıkıntılı Solunum Sendromu (ESSS)'nun mortalitesi halen yüksektir. Klinik bir antite olan ESSS'nύn histopatolojik karşıtı Difüz Alveol Hasarı- (DAH)'dır. Çalışmada amacımız yoğun bakım ünitelerinde tedavi gören ve Adli Tıp Kurumu Morg İhtisas Dairesinde otopsileri yapılan olgularda akciğerlere ait histopatolojik özellikleri değerlendirmek, akciğerlerdeki akut akciğer hasarının (AAH) varlığı ve derecesini araştırmaktır. Çalışmada Adli Tıp Kurumu Morg İhtisas Dairesi'nde 1999-2004 yıllarını kapsayan dönemdeki 18.508 adet otopsi raporu incelenerek yoğun bakım ünitesinde tedavi gören 77 olgu belirlendi. Olgulara ait yaş, cins, yoğun bakımda yatış süresi, yoğun bakıma yatış nedenleri, akciğerlerin histopatolojik tanıları ve ölüm nedenleri sınıflandırıldı. Olguların 12'si kadın, 65'i erkekti (K/E:1/3.6). Olguların yaşları 2-79 arasında değişmekte olup, ortalama yaş 34.9 idi. Olgular yoğun bakımdaki yatış süreleri açısından değerlendirildiğinde 24 olguda yaşam süresi 0-3 gün, 21 olguda 4-7 gün, 32 olguda 8 gün ve üzerindeydi. Olguların hastaneye yatış öyküsü; 66 olguda travmatik nedenlere, 3 olguda intoksikasyona, 8 olguda ise doğal nedenlere bağlı idi. Akciğerlerin morfolojik değerlendirilmesinde 31 olguda ağır derecede AAH (21 olgu eksüdatif dönem, 10 olgu proliferatif dönem), 6 olguda orta derecede AAH, 24 olguda hafif derecede AAH saptandı.10 olguda AAH olmaksızın pnömoni saptandı. 2 olguda nekrotizan granülomatöz iltihap (tüberkülozla uyumlu), 4 olguda ise nonspesifik bulgular tespit edildi. Olguların yatış süreleri, yatış nedenleri ve AAH'nın derecesi arasındaki ilişki istatistiksel olarak değerlendirildi. Spearmann korelasyon testi ile AAH'nın ağırlığı ile yatış süreleri arasında zayıf anlamlı ilişki saptandı.
Abstract
The high hospital mortality of Adult Respiratory Distress Sydrome (ARDS) has not diminished over the last years despite improvements in supportive intensive care. Diffuse alveolar damage (DAD) is the histopathologic finding that corresponds to the clinical entity of ARDS. The aim of this study is the histopathologic evaluation of the lungs in cases which were treated in intensive care units (ICU) and autopsied in the Council of Forensic Medicine. In reviewing the autopsy reports from the period 1999-2004 (18.508 cases), 77 cases hospitalized in ICU were selected. Classification was based on criteria like age, gender, duration of intensive care treatment, histopathologic assessment of lungs as well as the cause of death. Twelve of the cases were female and 65 male (F/M:1/3.6) with a mean age of 34.9 years ranging from 2 to 79 years. Hospitalization was necessary because of trauma in 66 cases, intoxication in 3 and natural reasons in 8 cases. Lung histopathology revealed severe APD in 31 cases, APD of moderate degree in 6 cases, and slight APD in 24 cases. 10 cases displayed pneumonia without APD. Necrotizing granulomatous inflammation was found in 2 cases, and in 4 cases showed nonspesific findings. The relationship between the duration and cause of hospitalization and the degree of APD was evaluated statistically. There was a weak correlation between APD and the hospitalization period according to the Spearmann correlation test.
