Özet

Bu makale Türkiye'de sağlık alanında özellikle 2000'li yıllarda hızlanan piyasa-yönelimli dönüşümü ele almaktadır. Bu dönüşüm, sağlığın esas olarak bir kamusal hizmet olarak görüldüğü önceki dönemlerin politika ve kurumlarının kapsamlı bir yeniden yapılanmasını gerektirmiştir. Yaşanan dönüşümün aktörleri ise devlet, Dünya Bankası başta olmak üzere ulusötesi kuruluşlar ile yerli ve yabancı sermayelerdir. Gerek kamusal alanda kalmaya devam eden kısmıyla sağlık hizmetleri sunumunun giderek daha fazla piyasa mantığı içinde verilmesi, gerekse de Türkiye'de çok sayıda özel sağlık kuruluşunun yurtiçi ve uluslararası ölçekte yatırımlarla büyümeleri sağlık alanında ticarileşmenin çeşitli veçheleridir. Çalışmada, devlet ve özel sağlık kuruluşları arasında hizmet sunum ilişkisinde yaşanan dönüşüm ve teşvik düzenlemeleri aracılığıyla, devletin gözetimi altında özel hastane sektörünün inşası gözler önüne serilmektedir. Bu amaçla, bir kısmı diğer sektörlerde de faaliyet gösteren çeşitli sermaye gruplarının sağlık alanında büyümek için kullandıkları stratejiler ve finansman kaynakları değerlendirilmektedir. Ayrıca, özel hastaneciliğin gelişiminin eğitim, turizm, finans, tıbbi cihaz, ilaç vb. ilişkili sektörlerdeki büyümeye ivme kazandırarak, ekonomiye dair geniş yansımaları bulunduğu vurgulanmaktadır.

Abstract

This paper addresses the market-oriented transformation of Turkish health industry which has been accelerated particularly in the 2000s. This transformation has required a comprehensive restructuring of the policy and institutions of the previous periods in which health was seen mainly as a public service. Among the actors of the transformation, there have been the state, international institutions such as the World Bank and domestic and foreign capital. Those health services which still remained in the public domain have been increasingly delivered in line with market principles. As the other facet of the commercalization of health services in Turkey, many private health institutions continue to expand via domestic and overseas investments. This paper explains the building of the private hospital industry under the state supervision on the basis of subsidies as well as the changing relationship between the state and private health institutions regarding service delivery. On this ground, the study addresses the strategies and financial sources that have been utilized to expand in the health sector by various capital groups, some of which are also active in other industries. The paper also stresses that the development of the private hospital sector has wider ramifications in the economy as it gives impetus to growth in related industries such as education, tourism, finance, pharmaceutical and medical equipment.
