Özet
Özel sağlık sektörü, basit bir kar-zarar ikiliği içinde değerlendirilemez. Sektörün değerlendirilmesi esnasında ekonomik olduğu kadar, sosyolojik olgular da göz önünde tutulmalıdır. Özellikle devlet aygıtı ve toplumsal sınıflar arasındaki ilişki düşünüldüğünde iki unsur belirginleşir: İlki, sermeye birikiminin sürdürülebilirliği açısından canlı emeğe yönelik biyolojik ve fizyolojik "yardımlar" olarak sağlık. Yani emek ve sermaye açısından ihtiyaç kategorisinde sağlığın konumu. İkincisi ise, sağlık üzerinden sisteme aktif ve pasif rızanın sağlamasında yeni politik teknolojilerin geliştirilmesi olarak sağlık. Bu kısa yazı, özel sağlık sektörünü analiz etmeden önce, bu iki özelliğe Marksist değer teorisi içinden cevap üretemeye çalışmıştır. Daha sonra, kamusal sağlığın tasfiyesi ve özel sağlık sektörünün büyümesinde, sektörü ampirik boyutta da işlemiştir. 1990 ve 2009 yılları arasında Türkiye'de özel sağlık sektörünün gelişimi ele alınarak, özellikle Sağlıkta Dönüşüm süreci sonrası yaşanan gelişmeler, rakamlar düzeyinde serimlenmiştir. Yazıda son olarak, özel sağlık sektörünün Türkiye için güncel veçhesi sayılabilecek sağlık turizmine değinilmiştir. Kuramsal ve olgusal boyutta özel sağlık sektörüne bakıldığında, mevcut yapısal koşullar altında geriye dönüşü olmayan bir yola girildiği anlaşılmaktadır.
Abstract
Private health sector can not be evaluated within a simple profit-loss dichotomy. During the evaluation of this sector, economic and sociologic facts should both be considered. Especially when thinking about the relationship between the state apparatus and the social classes, two factors become clear: first, health as a biological and physiological "assistance" directed to the dynamic labour in terms of the continuation of capital accumulation. That is, the state of health in demand category within the sense of labour and capita. Second, health as the development of new political technologies in providing active and passive consent for the system. This short article tries to find answers to these two aspects through Marxist value theory before analysing private health sector. Then, it discusses the sector in an empirical dimension within the contexts of the elimination of public health and the development of private health sector. The processes seen especially after the Transformation in Health duration are displayed at numeral levels by considering the development of private health sector in Turkey between 1990 and 2009. And finally, health tourism that can be thought as a current aspect of the private health sector for Turkey is dealt. When looking at the private health sector in an institutional and factual dimension, it is understood that we are in an irreversible way under these current structural conditions.
