Preemptif neostigmin uygulamasının abdominal histerektomi sonrası hasta kontrollü epidural analjezide etkinliği

Özet
Amaç: Çalışmamızda, neostigminin preemptif olarak epidural kullanımının poslflperatif analjezik etkinliğini araştırmayı amaçladık. Gereç ve Yöntem: Hastane Etik Kurulu onayıyla abdominal histerektomi geçirecek ASA I-II grubunda, 40-65 yaş arasında 45 olgu çalışmaya alındı. İndüksiyondan 20 dk önce epidural yolla Grup l' e 10 ml senim fizyolojik, Grup II'ye 0.5 mg neostigmin, Grup III'e 0.5 mg neostigmin+25 mg bupivakain verildi. Olgulara standart genel anestezi uygulandı. Peroperatııar monitör izasyonda ortalama arter basıncı (OAB), kalp atım hızı (KAH), per if eni oksijen saturasyonu kaydedildi. Postoperatif ağrı tedavisi hasta kontrollü epidural analjeziyle sağlandı. Postoperatif 1.,2., 6., 12., 24. saatlerde OAB, KAH, Visuel Analog Skala (VAS) ile ağrı takipleri yapıldı, 24 saatlik total analjezik tüketimi, yan etkiler kaydedildi. İstatistiksel değerlendirmelerde student, t, ANOVA testleri kullanıldı. P<0.05 anlamlı olarak değerlendirildi. Bulgular: Olguların per ve postoperatif OAB, KAH değerleri arasında anlamlı fark saptanmadı. VAS, 24 saatlik total analjezik tüketimi, yan etki sıklığı Grup III' de diğer iki gruba göre anlamlı olarak düşük bulundu. Sonuç: Preemptif amaçla epidural yolla uygulanan neostigminin bir lokal anestezik olan bupivakain ile kombine kullanıldığında postoperatif ağrıyı ve analjezik tüketimini anlamlı ölçüde azalttığım saptadık. Abdominal histerektomilerde postoperatif analjezik etkinlik için neostigmin+lokal anestezik kombinasyonunun iyi bir tercih olabileceği kanısına vardık.
Abstract
Objective: In our study we aimed to evaluate the postoperative analgesic efficacy of preemptive epidural neostigmine. Study Design: After the approval by the Medical Ethic Committee, 45 patients in ASA I-II aged between 40-65 years were included into the study. 10 ml 0.9% NaCl solution in Group I, 0.5 mg neostigmine in Group II, 0.5 mg neostigmine+25 ing bupivacaine in Group III were administered epidurally 20 minutes before induction. Standard, general anaesthesia was applied to the patients. Mean arterial pressure (MAP), heart rate (HR), peripheral oxygen saturation were recorded peroperatively. Patient controlled epidural analgesia was used for postoperative pain management. MAP, HR, pain evaluation with Visual Analogue Scale (VAS), adverse effects at postoperative 1., 2., 6., 12., 24. hours, total analgesic consumption for 24 hours were recorded. Student's t test and ANOVA were used for the statistical analyses. P<0.05 wax considered significant. Results: MAP, HR values were not significantly different per and postoperatively. VAS, total analgesic consumption and frequency of adverse effects were lower in Croup III when compared with other two groups. Conclusion: Preemptive epidural neostigmine decrease postoperative pain and analgesic consumption when combined with bupivacaine. We suggest that, neostigmine and local anaesthetic combination can be a good choice (for postoperative analgesia) after abdominal hysterectomies.
