TÜRKİYE’DE HALK SAĞLIĞI SOSYAL HİZMET UYGULAMALARININ İNCELENMESİ

Özet
Amaç: Bu araştırma, Türkiye’de halk sağlığı sosyal hizmeti (HSSH) uygulamalarının mevcut işlevselliğini, halk sağlığıkurumlarında çalışan sosyal çalışmacıların görüşleri çerçevesinde saptamak amacıyla yapılmıştır. Yöntem: Bu kesitsel,niceliksel araştırmada uygunluk örneklemi kullanılmıştır. Halk Sağlığı Genel Müdürlüğü’ndeki 167 sosyal çalışmacıyla anketkullanılarak görüşülmüştür. Veriler SPSS’te analiz edilmiştir. Bulgular: Katılımcıların %51.5’i erkek ve yaş ortalaması 28.74’tür.Yalnızca %17’sinin yüksek lisans derecesi varken, geri kalanı sosyal hizmet lisans derecesine sahiptir. %25.7’si lisansta HSSHile ilgili ders almış; %10.8’i halk sağlığı alanında staj yapmıştır. Halk sağlığı ilkelerine katılım puan ortalamaları 65 üzerinden56.13 (±5.74)’tür. %83.2’si “eğitimci” rolündedir. Ekip çalışma sıklıkları yüksektir.%18’inin halk sağlığı uzmanlarıyla ekipçalışması yürüttüğü saptanmıştır. %85’i ruh sağlığında uygulama yapmakta olup; en sık hizmet verilenler ruh sağlığı sorunuolanlardır. %89.2’sine göre en ciddi halk sağlığı sorunu bağımlılıktır. Uygulayıcılar, tütün / alkol / madde bağımlılığının ülkedesürekli artan ve en ciddi halk sağlığı sorunu olduğunu bildirmiştir. Halk sağlığı temel standartlarını yerine getirme puanları ortadüzeydedir 44.74 (±4.24, 70 üzerinden). Sosyal çalışmacıların, halk sağlığı sosyal hizmet temel standartları puan ortalamalarıile çeşitli değişkenler arasında anlamlı ilişkiler tespit edilmiştir (p<0.05). Sonuç: Araştırma Türkiye’de HSSH uygulamalarını elealan ilk kapsamlı çalışmadır. Araştırma kapsamında, halk sağlığı hizmetleri sunumunda, HSSH’nin önemli bir role sahip olduğu,halk sağlığı alanında çalışan sosyal çalışmacıların halk sağlığı alanı ile bir bütünleşme sağladıkları ve HSSH’yi geliştirmeyeçalıştıkları; öte yandan HSSH’nin Türkiye’de henüz gelişmekte olduğundan belirli standartlaşmalarda, yasal düzenlemelerdeve akademik çalışmalarda eksiklikler olduğu sonucuna ulaşılmıştır.
Abstract
Objective: This study examined the current functioning of public health social work (PHSW) practices in Turkey within the framework of the views of social workers working in public health institutions. Method: The convenience sampling method was used in this cross-sectional quantitative study. 167 social workers working in various governmental public health agencies and in different cities participated in the study. The data were analyzed using SPSS. Results: %51.5 of the participants was male and the mean age was 28.74. Only 17% were had MSW degrees and the rest of them had a bachelor degree of social work. 25.7% of them took courses on PHSW during their undergraduate education, and 10.8% had an internship in the public health field. The average score of the recognizing public health principles of participants was 56.13 (±5.74) out of 65 points. 83.2% of the participants are in the role of "educator". Teamwork frequency was found to be high. 18% of them have conducted teamwork with public health professionals. 85% was found to be practicing mental health; the most frequently served target population is those who have mental health problems. 89.2% of the participants reported that the most serious public health problem is the addiction. Practitioners reported that the tobacco/alcohol/substance addiction was the constantly increasing and the most serious public health issue in the Country. The mean score of practitioners fulfilling basic standards of public health was at the moderate level (44.74±4.24 out of 70). Significant correlations were found between the mean scores of the PHSW basic standards scores of the social workers and various variables (p<0.05). Conclusions: This research is the first comprehensive study to address the PHSW practices in Turkey. The study has great precaution because it reveals the current state of PHSW. In the study, the public health service presentation, led by the public health experts, has reached the conclusion that PHSW has a key role and that social workers are trying to develop PHSW that they integrate with the public health domain.
