Distal hipospadias onarım deneyimlerimiz

Özet
Amaç: Hipospadias external meanın penisin ventraline açıldığı bir patolojidir. En sık distal yerleşim gösteren ve 300 erkek doğumda bir görülen bu konjenital anomalinin tedavisi cerrahidir. Bu amaçla pek çok cerrahi yöntem tanımlanmıştır. Bu çalışmada kendi sonuçlarımız sunulmuştur. Gereç ve Yöntem: Distal hipospadias onarımı yapılan 74 hasta retrospektif olarak değerlendirildi. 11 MAGPI, 18 Mathieu, 44 TIPU ve 3 hastada Onlay ada flebi teknikleri uygulandı. Bu çalışmada olgularımızdaki komplikasyon ve başarı oranları değerlendirildi. Bulgular: MAGPI tekniği uygulanan 1 hastada darlık gelişti. Mathieu tekniği uygulanan 2 vakada tüp kaybı, 5 vakada fistül ve 1 olguda darlık gelişti. TIPU uygulanlarda ise sadece 3 olguda meatal darlık ve 1 olguda fistül gözlendi. Sonuç: Distal hipospadias onarımında pek çok değişik teknik uygulanmaktadır. Bunlar içerisinde TIPU yönteminin uygulaması kolay, daha anatomik bir yaklaşım sağlaması ve daha az komplikasyonları olması nedeni ile diğer yöntemlere tercih edilebileceğini düşünüyoruz.
Abstract
Objective: Hypospadias is a congenital anomaly seen one in 300 male birth and characterized by localisation of the external meatus over ventral part of the penis, mostly of the distal part. Treatment is surgical and many methods were described. Methods: 76 patients with distal hypospadias repair were retrospectively evaluated. The used methods and number of patients were as follows, MAGPI 11, Mathieu 18, TIPU 44, and Onlay flab 3, complications and succes rates were evaluated . Results: In one patient with MAGPI procedure stenosis developed. With Mathieu procedure 2 cases developed tube lost, 5 cases developed fistulas and 1 case developed stenosis. With TIPU methods only three meatal stenosis and one fistula were observed. Conclusion: Many different techniques are used for distal hypospadias repair. Among them TIPU is due to its ease, less complications rate and more anotomical approach.
