Multipl miyeloma ve sistemik lupus eritematozus: Bir olgu sunumu

Özet
Multipl miyeloma (MM) ve sistemik lupus eritematozus (SLE) birlikteliği çok nadir olarak bildirilmiştir. 20 yaşındaki kadın saç dökülmesi, soğukta el ve ayaklarda morarma, tekrarlayan eklem ve bel ağrısı şikayetleriyle kliniğimize başvurdu. Tetkiklerinde sedimantasyon: 96 mm/saat, Anti nükleer antikor 1:1000 granüler pozitif, C3 ve C4 düşüktü. Hastanın vertebra MR tetkikinde spinal korda bası yapan kitleler ve batın tomografisinde dalak alt pol komşuluğunda 5x2.5 cm çapında kitle lezyon saptandı. Batındaki kitle ve kemik iliği biyopsisinde malin plazma hücre infiltrasyonu tespit edildi. Bu bulgularla hastaya SLE, MM ve ekstramedüler plazmasitom tanısı kondu. Sonuç olarak SLE’li hastalarda monoklonal gammopati tespit edildiğinde MM şüphesiyle etraflıca araştırılması gerektiğini düşünmekteyiz.
Abstract
Multiple myeloma (MM) associated with systemic lupus erythematosus (SLE) has been rarely reported. 20-year-old female was admitted to our clinic with complaints; hair loss, arthralgia, recurrent back pain and discoloration of the fingers and toes when exposed to cold. Laboratory findings were as follows; sedimentation rate: 96 mm / h, anti nuclear antibody positive (granular 1:1000) and C3 and C4 was low. In the spinal MR, the mass lesions pressuring on the spinal cord were found. In the abdominal BT, 5x2.5 cm mass lesion was detected in the below spleen. The plasma cell infiltration was found in the abdominal mass and bone marrow biopsy examination. With these findings, SLE, MM and ekstramedüler plazmasitom were diagnosed. As a result, when monoclonal gammopati is detected in patients with SLE, MM should be considered.
