Nöroşirürji polikliniğine başvuran bel ve/veya bacak ağrılı hastaların değerlendirilmesi

Özet
Amaç: Nöroşirurji polikliniğine 1995 ile 2002 yılları arasında baş vuran hastaların retrospektif olarak tanıları ve tedaviye verdiği cevap incelendi. Gereç ve yöntem: Çalışma 12254 hasta içinde, bel ve/veya bacak ağrısı yakınması olan 5112 hasta üzerinde yapıldı. Hastaların yaşı, cinsiyeti, başvuru ayı, travma varlığı, ağrı süresi ve cerrahî endikasyon konulup opere edilenlerin tedavi sonuçları değerlendirildi. Bulgular: Polikliniğe baş vuran 12254 hastadan 5112 (%41.7)' sinde bel ve/veya bacak ağrısı mevcuttu. Bel ve/veya bacak ağrısı olan olgular incelendiğinde; yaş ortalaması 32.4 bulundu. Olguların 2250 (%44)' si bayan, 2862 (%56)' si erkekti. En fazla başvurunun Haziran ayında (%11) olduğu görüldü. Ağrının süresi açısından değerlendirildiğinde %52'sinin 12 aydan fazla süredir şikayetlerin olduğu tespit edildi. Olguların sadece % 24'ünde travma sonrası şikayetler başlamıştı. Hastaların 117 (%2.3)' si lomber disk hernisi tanısı ile cerrahi endikasyon konulup öpere edildi. Kalan diğer hastalar; lomber disk hernisi dışında cerrahi uygulanan, konservatif tedavi uygulanan veya fizik tedavi ve rehabilitasyona sevk edilen hastalardı. Lomber disk hernisi tanısıyla cerrahi uygulanan hastaların sonuçları değerlendirildiğinde; 27 hastada mükemmel, 84 hastada iyi, 5 hastada orta, 1 hastada kötü olarak tespit edildi. Sonuç: Bel ve/veya bacak ağrı hastaların yaklaşık %2' si lomber disk hernisi tanısı ile cerrahiye gerek duyar. Bizim hastalarımızda %2.3 oranında cerrahi gerekli oldu. Bu öpere edilen hastaların %95' i cerrrahi tedaviden fayda görmüştür. Cerrahi endikasyonu, doğru konulan lomber disk hernili hastalarda cerrahi tedavi faydalı olmaktadır.
Abstract
Objective: This study has been conducted on 5112 patients out of 12254 who referred to neuro-surgery polyclinic between the years of 1995 and 2002 on complaints of back and / or leg pains. Methods: The prognostic evaluations of those patients who underwent operation has been made together with a comparison arid analysis of age, gender, month of referral, the magnitude of trauma and the duration of pain. Results: Of the 12254 patients (who referred to the polyclinic 5112 ( 41.7%) had back and /or leg pains. Female patients comprised 44% of these patients; being 2250 in number; and the males comprised 56% of these patients, being 2862 in number; the mean age was found as 32.4. Month of June was found to be most frequent for those cases to refer to the hospital (11%). When we eva-luated the cases in terms of the duration of pain 52% of them had complaints for more than 12 months. Only 24% of the cases produced post-trauma complaints. 117 (2.3%) of the cases were the diagnosed and detected to have had lumbar disc herniations and thus they underwent operation. The rest of the patients were composed of the individuals who underwent operation other than lumbar disc herniation and who were applied to conservative treatment as well as those who were referred to physical treatment and rehabilitation. When we evaluated the results of the treatment designed for those cases undergoing herniated lumbar disc operation, perfect recovery in 27 patients, moderate in 5 patients and bad results in 1 patient were determined. Conclusion: In the final course of our study we came to the conclusion that of the patients suffering from back and / or leg pain and being diagnosed to have lumbar disc herniation 2% percent of them should undergo surgical treatment,which has been coherent to the results of our study as well as the previous ones.in our study 2.3% of those patients who underwent operation 95% produced good prognosis.We are of the belief that the right indications for surgery would be of greater benefit for the due treatment.
