Özet

Ateroskleroz çok sayõda genetik ve çevresel faktörün etkileşimiyle gelişen multifaktöriyel bir hastalõktõr. Plazma lipidleri ateroskleroz gelişiminde başlõca risk faktörleri arasõnda yer aldõğõndan lipoprotein sentez ve metabolizmasõnda yer alan genler ateroskleroza yatkõnlõkta önemli aday genlerdir. Plazma düşük yoğunluklu lipoprotein (LDL) düzeyi ateroskleroz gelişimi için önemli belirteçlerden biridir. LDL-reseptörü (LDLR) aterojenik LDL-kolesterolün kandan uzaklaştõrõlmasõnda önemli rol oynar. Bir serin proteaz enzimi olan Proprotein konvertaz subtilisin/keksin 9 (PCSK9) LDLR'nün translasyon sonrasõ degrasyonuna neden olarak kolesterol homeostazõnõ düzenlemektedir. PCSK9 genindeki bazõ mutasyonlar LDLR düzeyinde artma/azalma ile sonuçlandõğõ tanõmlanmõştõr. PCSK9 geninde fonksiyon kazanõmõ/kaybõna sebep olan mutasyon ve polimorfizmlerin hiperkolesterolemi/ hipokolesterolemi ile ilişkili olduğu bildirilmiştir. Son araştõrmalar aterosklerozda risk faktörü olarak LDL metabolizmasõ üzerindeki önemli etkileri nedeniyle PCSK9 gen varyantlarõ ve proteinfonksiyonlarõ üzerine yoğunlaşmõştõr.

Abstract

Atherosclerosis is a multifactorial disease resulted from the interaction of numerous genetic and environmental factors. Given that plasma lipids are major risk factors for atherosclerosis, genes involved in lipoprotein synthesis and metabolism are considered excellent candidate genes for susceptibility to atherosclerosis. The plasma low-density lipoprotein (LDL) level is the major determinant of the risk of atherosclerosis. The LDL receptor (LDLR) which is a surface membrane protein plays a major role in the removal of atherogenic LDLcholesterol particles from the blood. Proprotein convertase subtilisin/kexin 9 (PCSK9) is a secreted serine protease that regulates cholesterol homoeostasis by inducing post-translational degradation of LDLR. Several mutations have been identified in pcsk9 gene that result in increase/decrease LDLR level. Mutations in the PCSK9 gene have been associated with both hypocholesterolemia and hypercholesterolemia through 'loss-offunction' and 'gain-of-function' mechanisms, respectively. Current areas of interest and invastigation focus on gene variants and protein functions of PCSK9 enzyme as a risk factor for atherosclerosis because of its major effects on LDL metabolism.
