Short vertical incision in treatment of carpal tunnel syndrome; our clinical experience

Özet
Amaç: Çalışmanın amacı karpal tünel sendromunun (KTS) cerrahi tedavisinde kısa vertikal insizyonun etkinliğini değerlendirmekti. Gereç ve yöntem: Bu çalışmada, 2006 - 2009 yılları arasında tedavi edilen toplam 70 karpal tünel sendromlu olgu sunulmaktadır. Çalışmaya klinik ve elektrodiagnostik olarak KTS tanısı konmuş erken vakalar dahil edildi. Olgular yaş, cinsiyet, predispozan faktör, fizik muayene bulguları, tercih edilen cerrahi tedavi yöntemleri ve gelişen komplikasyonlar açısından incelendi. Tüm vakalarda açık cerrahi yaklaşım kısa vertikal insizyon ile uygulandı. Bulgular: Yaş ortalaması 54 olan 70 olgunun 58ü kadın (%82.8) ve 12si erkek (%17.2) idi. KTS 6 vakada bilateral olup, 54 vakada sağ 10 vakada sol eldeydi. En sık predispozan faktör olarak, 50 olguda (%71.4) ellerin yoğun strese maruz kalması ile karşılaşıldı. En sık gözlenen semptom 65 olguda (%92.8) nokturnal parastezi idi. Tinel ve falen testlerinin duyarlılıkları sırasıyla %56 ve %62olarak bulundu. Hastaların ortalama takip süresi 3 yıl idi. Kısa vertikal insizyon uygulanan vakalarda rekurens, iyileşme süresi ve postoperatif ağrı bakımından oldukça tatmin edici sonuçlar elde edildi. Sonuç: Kısa vertikal insizyon tekniği özellikle erken vakalarda uygulaması kolay ve etkili bir yöntem olarak bulundu.
Abstract
Aim: The aim of the study was to evaluate the results of short vertical incision in treatment of Carpal tunnel syndrome (CTS). Materials and method: 70 cases of CTS treated with the short vertical incision between the years of 2006-2009 were presented. CTS was bilateral in 6 cases, the right hand in 54 and the left hand in 10 cases. The cases diagnosed CTS in both clinically and electrodiagnostically were reviewed regarding the age, sex, predisposing factors, physical examination findings, and complications. Results: 58 of the cases were female (82.8%), and 12 were male (17.2%). Mean age was 54. The most common predisposing factor was the exposure of hands to intense distress in 50 cases (71.4%). The most common observed symptom was nocturnal paresthesia in 65 cases (92.8%). The sensitivity of tinnel and phalen tests were 56% and 62% respectively. The mean postoperative follow-up was 3 years. Quite satisfying results were obtained in cases which were applied the short vertical incision as regards recurrens, recovery time and pain postoperatively. Conclusion: Consequently the short vertical incision technique was found to be simply and efficacious in especially early cases.
