Testing for induction of clindamycin resistance in erythromycin-resistant isolates of coagulase negative staphylococci and Staphylococcus aureus

Özet

Amaç: Bu çalışmada, İstanbuldaki 1000 yataklı üçüncü basamak bir hastanede izole edilen hastane kaynaklı stafilokok suşları arasında izlenen indüklenebilir klindamisin (KLİ) direnç fenotiplerinin ve sıklığının belirlenmesi amaçlanmıştır. Gereç ve Yöntem: Çalışma kapsamına, ardışık klinik örneklerden elde edilmiş olan 23 (%33.3)ü metisiline dirençli 69 Staphyloccus aureus ve 44 (%37.9)ü metisiline dirençli 116 koagülaz-negatif stafilokok (KoNS) izolatı alınmıştır. Bulgular: Disk difüzyon testlerinde, iki indüksiyon (D+ve D) (indüklenebilir makrolid-linkozamid streptogramin B [iMLSB] direnci) ve üç non indüksiyon fenotip varlığı saptandı. KLİ diski çevresinde temiz D-zonu (D fenotipi), dört (%2.2, n=185) izolatta; iç kısımda küçük koloniler içeren D-zonu (D+ fenotipi) ise 20 (%10.8, n=185) izolatta tespit edildi. İndüklenebilir olmayan KLİ duyarlı ve eritromisin (ERT) dirençli fenotip (M/MSB tip fenotip), dokuz (%4.9, n=185) izolatta rastlandı. Disk difüzyon testinde, 67 (%36.2, n=185) izolatın KLİ ve ERT disklerinin her ikisine duyarlı; 85 (%45.9, n=185) izolatın ise dirençli (konstitütif MLSB [cMLSB] direnci) olduğu anlaşıldı. Çalışmamızda MLSB direnci (cMLSB [n=85] + iMLSB [n=24]), 109 (%58.9) izolatta saptanmıştır. Disk difüzyon testinde KLİ indüksiyon sonuçlarımız, iki fenotip göstermiştir. D-zon fenotip (D ve D+ ), 24 (%13.0, n=185) izolatta izlenmiştir ve KLİ indüksiyonu (iMLSB direnci) pozitif olarak değerlendirilmiştir. Sonuç: Stafilokok izolatlarının indüklenebilir KLİ direnci yönünden rutin testinin yapılması, CLSI tarafından 2008 kılavuzunda önerilmektedir ve tedavide başarısızlık yaşanacak özellikle iMLSB izolatları ile infekte hastalar için faydalı olacaktır.

Abstract

Objective: The aim of this study was to determine the incidence and the phenotypes of inducible clindamycin (CLI) resistance among nosocomial staphylococci in a 1000-bed tertiary hospital at Istanbul, Turkey. Medhods: Coagulase-negative staphylococci (CoNS) and Staphyloccus aureus isolates obtained from consecutive clinical specimens were included in the study, comprising 116 CoNS isolates, 44 (37.9%) of which were methicillin-resistant; and 69 S.aureus isolates, 23 (33.3%) of which were methicillin resistant. Results: Two induction (D+ and D) phenotypes (inducible macrolide-lincosamide-streptogramin B [iMLSB] resistance) and three non-induction phenotypes were detected in disk diffusion tests. A clear D-shaped zone (D phenotype) growing up to CLI disk was found in four (2.2%, n=185) isolates and a D-shaped zone containing inner small colonies (D+ phenotype) was found in 20 (10.8%, n=185) isolates. Erythromycin (ERY) resistance and CLI susceptible phenoype were found in nine (4.9%, n=185) isolates and they were not inducible (M/MSB type phenotype). Isolates susceptible by diskdiffusion testing to both CLI and ERY were found in 67 (36.2%, n=185) strains and contrary to both CLI and ERY resistance was found in 85 (45.9%, n=185) isolates (constitutive MLSB [cMLSB] resistance). In our study, MLSB resistance (cMLSB [n=85] + iMLSB [n=24]) was detected in 109 (58.9%) isolates. The Dzone phenotype (D and D+ ), observed in 24 (13.0%, n=185) isolates and considered positive for CLI induction (iMLSB resistance). Conclusion: Routine testing of staphylococcal isolates for inducible CLI resistance is recommended in the CLSI 2008 guidelines and help to choose treatment failures especially for patients infected with iMLSB isolates
