Özet
Amaç: Bu çalışmanın amacı tetik parmak konservatif tedavisinden fayda görmemiş hastalarda açık cerrahi tedavi sonuçlarının değerlendirilmesidir. Metod: Ocak 2006–Ocak 2011 tarihleri arasında 215 tetik parmak tedavi edildi. Tek doz kortikosteroid enjeksiyonundan fayda görmemiş 152 tetik parmakta (142 hasta (42 erkek (%29.6), 100 kadın (%70.4)) lokal anestezi altında açık cerrahi gevşetme yapıldı. Hastaların hepsinde A1 puley üzerinde hassasiyet ve çoğunda da nodül mevcuttu. Tetiklenme tüm hastalarda mevcuttu. 75 hastada sağ 77 hastada ise sol eldeydi. Ortalama yaş 40.26 olarak bulundu (yaş aralığı 0.7- 79). Sonuçlar: Tek doz kortikosteroid enjeksiyonun başarı oranı %30.7 (63 hasta 63 tetik parmak). Hastaların %69.3’üne lokal anestezi altında açık cerrahi gevşetme uygulandı. Hastalardan yetersiz gevşetme yapılan 1’i dışında sonuçlar mükemmeldi ve lokal anestezi yada cerrahiye bağlı komplikasyon olmadı. Hastaların hepsinde ağrı geçti. Rekürrens saptanmadı Tartışma: Sonuç olarak, lokal anestazi altında tetik parmak açık cerrahi gevşetmesinin etkili, güvenli ve kolay bir teknik olduğunu düşünüyoruz. Konservatif tedavinin başarısız olduğunda, tedavi sürecinde ilk tercih olabilir.
Abstract
Objectives: The aim of this study is to evaluate the results of open surgical release after failed conservative treatment of trigger finger and thumb. Method: Between January 2006 and January 2011, 215 trigger fingers were treated. Open surgical release under local anaesthesia was performed in 152 trigger fingers (142 patients (42 male (29.6%), 100 female (70.4%)) in which one dose of corticosteroid injection failed to treat the disease. All patients had tenderness over the A1 pulley and most of them had a nodule. Triggering was clearly observed in all patients. Seventy-five trigger fingers (49.3%) were in right hand whereas 77 (50.7%) were in left hand. The average age was 40.26 (range 0.7- 79 years). Results: The success rate of one-dose corticosteroid injection was 30.7% (63 patients with 63 trigger fingers). 69.3% of patients (142 patients with 152 trigger fingers) received open surgical release under local anesthesia. All of surgical releases were successful except one insufficient release which was repaired by revision and we did not have any complications due to local anesthesia and the procedure itself. All patients had complete pain relief. No recurrence was detected. Conclusion: In conclusion we think that, open surgical release for trigger finger under local anesthesia is an effective, safe and easy technique. When conservative treatment fails, it can be the first choice for the course of the treatment.
