KOAH’a bağlı akut solunum yetmezliğinde noninvazif mekanik ventilasyon uygulanması

Özet
Amaç: KOAH’a bağlı akut solunum yetmezliği ile başvuran hastalarda noninvazif pozitif basınçlı ventilasyonun (NIMV) etkinliğini standart medikal tedavi ile karşılaştırmaktır. Gereç ve Yöntem: Çalışmaya KOAH ve solunum yetmezliği tanılı toplam 92 olgu çalışmaya alındı. Standart medikal tedavi 34 olguya (Grup 1), NIMV + medikal tedavi (Grup2) 58 olguya uygulandı. Bulgular: Olguların yaş ortalamaları 63 idi. Grup 1’de, başlangıç ile tedavi sonundaki kan pH değerleri 7.33 ve 7,41 (p 0.05) idi. APACHE II skorları ise 11 ve 7 (p 0.05), APACHE II skorları karşılaştırıldığında grup 1 tedavi sonu APACHE II skoru ortalaması ile grup 2’nin APACHE II skoru ortalamaları arasındaki fark anlamlıdır (p 0,05). Her iki grubun mortalite oranları arasında istatistiksel olarak anlamlı farklılık saptanmadı. Sonuç: KOAH ve akut solunum yetmezliği olgularında noninvazif mekanik ventilasyon uygulaması hastanede kalış süresi ve APACHE II skorunda daha anlamlı bir düzelme sağlarken, entübasyon ve mortalite oranlarında istatistiksel olarak anlamlı olmayan bir düzelme sağlamıştır.
Abstract
Objective: The aim was to investigate the efficacy of noninvasive mechanical ventilation (NIMV) in COPD patients with acute respiratory failure. Methods: Study included 92 patients with a mean age of 61. Thirty-four cases received only standard medical treatment (Group 1). Fifty-eight cases received NIMV and standard medical treatment (Group 2). Results: In group 1, blood pH values were 7.33 and 7.41, p< 0.000. PaCO2 values were 56 mm Hg and 47 mm Hg, p<0.000 and APACHE II scores were 13 and 10, p< 0.000. In group 2, blood pH values were 7.34 and 7.42, p< 0.000. PaCO2 values were 58 mm Hg and 56 mm Hg, p> 0.05. APACHE II scores were 11 and 7, p<0.000. When both groups were compared, statistical significance in pH levels was not detected, p<0.001. Post treatment APACHE II score averages of the groups were significantly different. Treatment success rates were 82 % (28/34) in group 1, 89 % (52/58) The average length of hospital stay was 12 days for group 1 and 6 days for group 2. p<0.001. Mortality rates were 3/34 (8.8 %) for grup 1 and 1/58 (1.7 %) for group 2, p>0, 05. Conclusion: While in noninvasive mechanic ventilation applied COPD and acute respiratory failure, more significant recovery was observed in the average length of hospital stay and APACHE II scores compared with the standard medical treatment group, there was no important difference among the results of entubation and mortality although somewhat improvement in cases of NIMV.
