İleri evre küçük hücreli dışı akciğer kanserinde siklofosfamid, etoposid ve sisplatin kombine kemoterapisi

Özet

Amaç: Sitotoksik kemoterapi, küçük hücreli dışı akciğer kanseri (KHDAK) ileri evre tedavisinde artan bir öneme sahiptir. İleri evre hastalıkta kemoterapinin sağkalım süresinin etkisini araştıran pek çok çalışma yayınlanmıştır. Gereç ve Yöntem: Biz çalışmamızda CEP (Siklofosfamid, Etoposid, Sisplatin) tedavisi alan ve çeşitli nedenler ile kemoterapi almayan KHDAK hastalarının sağkalım sürelerini karşılaştırmayı amaçladık. CEP tedavisinde oluşan toksisiteyi değerlendirdik. CEP tedavisi, evre IIIB ve evre IV KHDAK olan 37 hastaya (3 bayan, 34 erkek) verildi. Kemoterapi almayan evre IIIB ve evre IV KHDAK olan 13 hasta (l bayan, 12 erkek) kontrol grubunu oluşturdu. CEP grubunda hastalara 21 gün arayla siklofosfamid l g/m2, cisplatin 50mg/m2 ve etoposid 80mg/m2 kombine kemoterapisi uygulandı. Bulgular: Tam yanıt hiçbir olguda alınamadı, kısmi yanıt sekiz olguda (% 21.6), stabil yanıt 13 olguda (%35.1) ve progresyon 16 olguda (%43.2) saptandı. CEP tedavisinde medyan sağkalım süresi 5.7±4.2 ay ve medyan yanıt süresi 2.97±3.1 ay saptandı. CEP tedavisine bağlı bulantı-kusma 17 hastada (% 45.9), anemi 10 hastada (% 27) ve nötropeni l hastada (% 2.7) gelişti. Kemoterapi almayan kontrol grubunda ise medyan sağkalım süresi 4.5±3.2 ay olarak saptandı. Her iki grubun medyan sağkalım süreleri karşılaştırıldığında istatistik olarak anlamlı bir fark olmadığı görüldü (p>0.05). Her iki grupta bir yıllık sağkalım oranı sırasıyla n:37 (%8.9) ve n:13 (%7.6) idi. Sonuç: Sonuç olarak çalışmamızda elde edilen verilere göre yanıt oranlarının yapılan çalışmalardan farklı olmadığı, fakat sağkalım süresi ile bir yıllık sağkalım oranlarının çok düşük olduğu ve yan etki profilinin ise tolere edilebilir düzeyde bulunduğu gözlendi.

Abstract

Purpose: In this study, we aimed to compare the survivals of the patients with non-small cell lung cancer (NSCLC) treated with cyclophosphamide, etoposide, and cisplatinum (CEP) with that of controls who are not given chemotherapy. We have also evaluated the toxicity related to CEP treatment, CEP treatment were given to 37 patients (3 women, 34 men), stage IIIB or stage IV. Controls were consisted of 13 patients (1 woman, 12 men) stage IIIB or stage IV. Results: Combination chemoterapy of cyclophosphamide at dose of lg/m2, etoposide 80 mg/m2, and cisplatinum 50 mg/m2 were administered with 21 days intervals. Of 37 patients, 8 had partial response (21.6 %),13 had stable disease (35.1 %), and 16 had (43.2 %) progression. Median survival rate of the patients was 5.7±4.2 months (2 to 16 months). Median response rate was 2.97±3.1. Nause and vomiting were the most frequent side effects of chemotherapy in a rate of 45.9% (in 17 patients), followed by anemia 27 % (10 patients), and neutropenia 2.7% (1 patient). In control group median survival was 4.5±3.2 month and there was no statistically important difference between chemotherapy and control group. One year survival rates of the chemotherapy group and controls were 18.9% (n=37) and 7.6% (n=13), p>0.05. Conclusion: As a conclusion although we have found one year survival advantage for combination chemotherapy, but not statistically significant, there was no median survival advantage between combination chemotherapy with CEP and controls.
