Varikosel tanısında renkli doppler ultrasonografinin fizik muayeneye katkısının değerlendirilmesi

Özet

Amaç: Bu çalışmada varikosel düşünülen olgularda, varikosel tanısında basit fizik muayenenin değerini ve buna eklenen skrotal renkli dopler ultrasonografinin(RDUS) katkısını incelemekti. Materyal ve Metot: Üroloji polikliniğinde fizik muayenede varikosel saptanan hastalar çalışmaya alındı. Çalışmaya katılan tüm hastalar basit fizik muayene (BFM) ile değerlendirildi ve ardından tüm hastalara skrotal renkli dopler ultrasonografi (RDUS) yapıldı. Bulgular: Bu çalışmada varikosel saptanan 57 hasta retrospektif olarak değerlendirildi. Hastaların yaş ortalaması 28 idi. Hastaların rutin ürolojik muayenesi yapılıp ardından tüm hastalara skrotal renkli dopler ultrasonografi (RDUS) istendi. Fizik muayene ile toplam 57 hastanın 44ünde varikosel tespit edilirken, renkli dopler ultrasonografi ile 57 hastanın tümünde varikosel tespit edildi. Hem fizik muayenede hem de RDUSla tespit edilen olguların tümü sol varikosel idi. Varikosel tespit etme açısından fizik muayene ile renkli dopler arasında anlamlı fark tespit edildi(p<0,001). Fizik muayenede varikosel tespit edilemeyip, renkli dopler ultrasonografi ile varikosel tespit edilen olguların sayısı 13 idi. Fakat renkli dopler ultrasonografi ile tespit edilen olguların tamamı subklinik varikosel vakaları idi. Bu olguların hiçbirinde cerrahi tedaviye gerek duyulmadı Sonuç: Varikosel tanısında RDUSla BFMye göre daha çok olguya varikosel tanısı konmuştur. Fakat bu olguların hepsi subklinik varikosel olduğundan, bunlara herhangi bir cerrahi tedavi uygulanmamıştır. Dolayısıyla klinik öneme sahip varikosel tanısında, RDUSun BFMye bir katkı sağlamadığı görülmüştür.

Abstract

Aim: In this study in cases suspected of varicocele, the aim was to examine the value of simple physical examination (SPE) and the contribution of scrotal color doppler ultrasonography (CDU) that was added to this in the diagnosis of varicocele. Material and Methods: The patients found to have varicocele on physical examination in the clinic of urology were taken to the study. All of the patients participated in the study were evaluated with a simple physical examination (SPE), and then scrotal color doppler ultrasonography (CDU) was performed to all of the patients Results: 57 patients found to have varicocele were retrospectively evaluated in this study. Routine urological examination of the patients were performed and then scrotal color doppler ultrasonography (CDU) was requested for all of the patients.While varicocele was detected in 44 of totally 57 patients with physical examination, it was detected in all of 57 patients with color doppler ultrasonography. All of the cases detected both on physical examination and with CDU were left varicocele. Significant difference was found between physical examination and color doppler in terms of the detection of varicocele (P <0.001). The number of the cases that varicocele was not detected on physical examination but varicocele was detected with color doppler ultrasonography was 13. But all of the cases detected with color doppler ultrasonography were the cases of subclinical varicocele. Surgical treatment was not needed in none of these cases. Conclusion: In the diagnosis of varicocele compared to SPE, more diagnosis of varicocele was diagnosed with CDU in more cases. But since all of these cases have been subclinical varicoceles, any surgical treatment has not been applied to them. Therefore, it has been seen that CDU did not provide the contribution to SPE in the diagnosis of varicocele with clinical significance.
