Bölgemizde üriner enfeksiyona yolaçan patojenlere karşı gelişen antimikrobiyal direncin değerlendirilmesi

Özet
Amaç: Bölgemizde üriner enfeksiyona neden olan bakteriyel mikroorganizmaları ve değişen bakteriyel direnci araştırmaktı. Materyal ve Metot: 2008 ve 2010 yıllarında Bingöl Devlet Hastanesi Üroloji polikliniğine üriner enfeksiyon şikayetiyle başvuran 3352 hastadan gönderilen idrar kültürleri ve antibiyotik duyarlılıkları retrospektif olarak değerlendirildi. Çalışmaya erişkinler alındı. Çocuk hastalar ise (18 yaş altı) çalışma dışı bırakıldı. Bulgular: Hastaların yaşı 18 ile 86 arasında (ortalama 43,47±19.50) değişmekteydi. Toplam 3352 hastanın 1612 erkek, 1730u ise kadındı. ødrar kültürü gönderilen 3352 idrar örneğinden 290nında (%8) üreme oldu. ødrar kültüründe üreyen patojenler sırasıyla %69 E.coli, %10 klebsiella, %5 enterecoccus sp., %4 proteus, %3 enterobacter sp., %1 staphilococcus aureus, %1 pseudomonas aeruginosa ve %0.5 morgenalla üremiştir. Üreme tespit edilen kültürlerde direnç oranları sırası ile ampisilin için %35, amoksisilin klavulonik asid için %30, TMP-SMX için %48, seftriakson için %24, sefuroksim aksetil için %21, seftazidim için %19, sefoperozon-sulbaktam için %7, meropenem için %1 ve imipenem için %1, gentamisin için %9, amikasin için %5, siprofloksasin için %44, norfloksasin için %38 olarak tespit edildi. Sonuç: Bakteriyel ajanlar ve antibiyotik duyarlılıkları bölgeden bölgeye değişiklik göstermektedir. Bu nedenle mikroorganizmalara karşı bölgesel antibiyotik direncinin periyodik olarak araştırılması ve özellikle ampirik tedavi seçiminde bu bilgilerin dikkate alınması gerekmektedir. Bu durumda tedavideki başarı artacak, gereksiz antibiyotik kullanımı önlenecek ve antibiyotik direncindeki artış azalacaktır.
Abstract
Aim: Our aim was to investigate the bacterial microorganisms that cause the infections of urinary and changing bacterial resistance in our region. Material and Method: In 2008 and 2010,urine cultures which were sent from 3352 patients who presented with complaints of urinary infection to Urology Clinic of Bingol State Hospital and antibiotic sensitivities were retrospectively evaluated. Adults were enrolled to the study. Pediatric Patients (under 18) were excluded from the study. Results: The age of the patients varied between 18 and 86 (average 43.47 ± 19:50). Of totally 3352 patients,1612 were male and 1730 were female. Of 3352 urine cultures which were sent from urine samples,290 (8%) were positive. Pathogens which proliferated in urine culture were produced respectively 69% E. Coli, 10% Klebsiella, 5% enterecoccus sp., 4% Proteus, 3% Enterobacter sp., 1% staphilococcus aureus, 1% Pseudomonas aeruginosa and 0.5% morgenalla. In cultures in which reproductive were determined, the rates of resistance were detected to be respectively 35% for ampicillin, 30% for amoxicillin-clavulanic acid, 48% for TMP-SMX, 24% for ceftriaxone, for 21% cefuroxime axetil, 19% for ceftazidime , 7% for sefoperozon-sulbactam , 1% for meropenem and 1% for imipenem, 9% for gentamicin, 5% for amikacin, 44% for ciprofloxacin, 38% for norfloxacin. Conclusion: Bacterial agents and their antibiotic sensitivities vary from region to region. For this reason, periodic investigation of regional antibiotic resistance against the microorganisms and particularly in the selection of empirical treatment, these informations should be taken into account. In this case, the success in treatment will increase, the unnecessary use of antibiotics will be prevented and the increase in antibiotic resistance will decrease.
