Ekstrahepatik safra yollarının tıkanmalarında cerrahi tedavi metodları

Özet
Ekstrahepatik safra yollarında tıkanmaya neden olan hastalıkların tanı ve tedavisi çok çeşitlidir. Ekstrahepatik safra yolundaki tıkanma sonrası tıkanma ikteri adını verdiğimiz klinik tablo oluşur. Tanı fizik muayene, batın ultrasonografisi, batın tomografisi, magnetik rezonans kolanjiopankreatikografi (MRCP) ve endoskopik retrograd kolanjiopankreatikografi (ERCP) ile konur. Tanı yöntemlerindeki gelişmeler sayesinde tıkanıklığa neden olan patoloji ve tıkanıklığın yerinin belirlenmesi ile gerekli tedavi yöntemi tespit edilebilmektedir. Tıkanma sarılığına neden olan hastalıkların tedavisinde mortalite ve morbiditeyi artıracak gereksiz cerrahi girişimleri önlemek için, doğru tanı koymak ve uygun cerrahi işlemi uygulamak gerekmektedir. Böylece gereksiz cerrahi girişimler önlenebilmektedir. ERCP’nin terapötik kullanımı, açık kolesistektomi ve drenaj ameliyatlarının yerini laparoskopik kolesistektomi ve ERCP’nin almasına yardım etmiştir. ERCP uygulamasına ilave olarak MRCP’nin tanı yöntemi olarak kullanılmaya başlaması, tanısal ERCP uygulama oranlarını azaltmıştır. Çalışmamızda eksrahepatik safra yollarında tıkanıklık sonucu tıkanma sarılığı gelişen 59 hastada anamnez, fizik muayene, laboratuar ve radyoloji bulguları incelenmiş ve tıkanıklığa neden olan patoloji tespit edilmiştir. Patoloji sonucuna göre uygulanan cerrahi metodlar tartışılmıştır.
Abstract
Diagnostic and treatment options of the diseases causing obstructive icter in extrahepatic bile ducts are various. A clinical spectrum called obstructive icter develops after the obstruction has occured in extrahepatic bile ducts. The diagnosis was carried out via physical examination, abdominal ultrasound, abdominal tomography, MRCP and ERCP. In order to prevent unnecessary surgical interventions that increase morbidity and mortality rates in the treatment of obstructive icter, it is essential to diagnose the cases accurately and to apply most appropriate surgical procedures. With the help of the latest diagnostic developments, the pathology leading to the obstruction and the appropriate treatment regime could be decided via the determination of the obstructed area. Thus, unnecessary surgical interventions could be avoided. Therapatic use of ERCP has caused open cholecystectomy and drainage operations to be replaced by laparoscopic cholecystectomies and ERCP. The fact that MRCP was started to be used as a diagnostic criterion, as well as ERCP procedure, has decreased diagnostic ERCP procedure. In our study, such evaluations as physical examination, laboratory and radiologic findings were investigated, and the pathology leading to the obstruction was determined in 59 patients with obsruction icter developing due to obstruction in the bile ducts. Surgical methods performed according to pathological findings are reviewed in the study.
