Mesane tümörlerinin erken tanısında ve takibinde NMP22 bladderchek testinin yeri

Özet
Amaç: Mesane kanserinin erken tanı ve takibinde Nükleer Matriks Protein 22 Bladder Chek (NMP22BC) testinin kullanılabilirliğini incelemektir. Gereç ve yöntem: Bu çalışma Nisan 2003-Eylül 2005 tarihleri arasında hematürisi olan veya mesane kanseri tanısıyla izlenen 211 hasta üzerinde gerçekleştirilmiştir. NMP22BC tümör markırı ile sistoskopi karşılaştırıldı Bulgular: Bu çalışmada NMP22BC testinin hematüri grubunda sensitivitesi %73,3; spesifitesi %88,0; pozitif prediktif değeri %45,8; negatif prediktif değeri %94,9 olarak bulunurken; mesane tümörü grubunda sensitivite %100; spesifite %96; pozitif prediktif değeri %77 ve negatif prediktif değeri %100 olarak tespit edilmiştir. NMP22BC testinde dışlama kriterleri uygulanması etkinliğini daha da arttıracaktır. Sonuç: NMP22BC testinin hematürili hastaların ilk değerlendirilmesinde yol gösterici olacağı görülmüştür. Mesane tümörlü grupta elde edilen sensitivite ve negatif prediktif değerler, testin mesane tumor takibinde sistoskopiye alternatif olabileceğini ve sistoskopi sıklığını azaltabileceğini düşündürmektedir
Abstract
Aim: We investigated the availability of the NMP22 (Nuclear Matrix Protein) Bladder Check ® 22 in the early diagnosis and follow-up of bladder cancer. Material and Method: This study was carried out on 211 patients with hematuria or who were followed due to a diagnosis of bladder cancer, between April 2003 and September 2005. NMP22BC tumor marker and cystoscopy were compared with each other. Findings: Sensitivity, specificity, positive predictive and negative predictive values of the NMP22 BC testing were determined respectively, as 73.3%, 88.0%, 45.8% and 94.9% for the patients included in the Hematuria Group and respectively, as 100%, 96%, 77%, and 100% for the patients included in the Bladder Cancer Group. Application of the exclusion criteria in NMP22BC testing shall intensely increase efficacy. Result: We assumed that NMP22BC could be a guideline in the baseline assessment of patients with hematuria. Sensitivity and negative predictive values obtained from the Bladder Cancer Group makes us consider that the test mentioned above may serve as an alternative testing instrument against cystoscopy, during the follow up period of the bladder cancer and reduce the frequency of cystoscopy.
