Perioperatif miyokard enfarktüsü (Olgu sunumu)

Özet

Miyokard enfarktüsü perioperatif dönemde görülen en önemli komplikasyonlardan biridir. Elektif cerrahi planlanan, asemptomatik iskemik kalp hastalığı olan hasta preoperatif kardiyolojik açıdan orta riskli olarak değerlendirildi. Hastaya operasyon süresince anti-iskemik proflaksi uygulandı ve EKG değişikliği gözlenmedi. Postoperatif dönemde çekilen EKG'sinde ve kan biyokimyasında herhangi bir göğüs ağrısı tarif etmemesine rağmen miyokard enfarktüsü bulguları olduğu görüldü. Hasta heparinize edilip anti-iskemik tedavisi düzenlendi. Bu olgu sunumunda iskemik kalp hastalığı olan hastada uygun hazırlığa rağmen perioperatif dönemde gelişen miyokard infarktüsü sunulmuştur.

Abstract

Myocardial infarction is one of the most serious perioperative complications. Elective surgery was planned for a patient with asymptomatic ischemic heart disease, and the patient was evaluated as medium risk after preoperative cardiological consultation. During surgery, the patient received an anti-ischemic prophlaxis, and no any ECG changes were detected. Blood biochemistery and ECG findings were consistent with myocardial infarction in the postoperative period although the patient did-n't suffer any chest pain. Heparin was started following anti-ischemic treatment. We present a case of myocardial infarction in a patient with ischemic heart disease, in spite of proper anesthetic perioperative preparation.
