Unilateral spinal anestezide düşük doz bupivakaine eklenen fentanilin motor ve duyusal blok üzerine etkileri

Özet
Çalışmamızda alt ekstremite operasyonlarında düşük doz hiperbarik bupivakaine eklenen fentanilin motor ve duyusal blok üzerine etkisinin araştırılması amaçlandı. Kısa süreli, unilateral alt ekstremite operasyonu geçirecek, premedikasyonsuz, ASA I-II sınıfından 30 olgu rasgele 3 eşit gruba ayrıldı. Operasyon tarafı altta kalacak şekilde yan yatar pozisyonda 26G atravmatik iğneyle L2-3 aralığından spinal ponksiyon yapıldı. Grup I'e 5 mg %0.5 hiperbarik bupivakain, Grup II'ye 5 mg %0.5 hiperbarik bupivakain + 12.5 mg fentanil ve Grup III'e 5 mg %0.5 hiperbarik bupivakain + 25 mg fentanil 10 sn içinde intratekal uygulandı. Olgular 10 dakika yan pozisyonda tutulduktan sonra sırtüstü pozisyona çevrildi. Olguların preoperatif ve intraoperatif ortalama kan basınçları (OKB), kalp atım hızları (KAH) ve periferik oksijen satürasyonları (Sp02) kaydedildi. Her iki ekstremitede maksimum duyusal ve motor blok seviyeleri, anestezi yeterliliği, duyusal bloğun L1 dermatomuna gerileme ve postoperatif 24 saatteki analjezik ihtiyacı zamanları, yan etkiler ve komplikasyonlar kaydedildi. Her üç grupta da tüm ölçüm zamanlarında OKB; KAH ve Sp02 değerlerinde anlamlı fark gözlenmedi. Operasyon tarafında maksimum duyusal blok seviyeleri ise sırasıyla ortalama T11 (T8-L1), T9 (T3-12) ve T10 (T4-L1) iken, karşı tarafta ise her üç grupta duyusal blok gözlenmedi. Operasyon tarafında operasyon süresince Grup I'de 1, Grup II'de 4 olgu ve Grup III'te 6 olguda 3. derece motor blok gözlenirken, karşı tarafta hiç bir olguda motor blok gözlenmedi. İlk analjezik ihtiyacı ve duyusal bloğun L1 dermatomuna gerileme zamanlarında gruplar arasında fark gözlenmedi. Kaşıntı, titreme, bulantı-kusma ve idrar retansiyonu gibi yan etkiler en çok Grup III'te gözlendi. Bu çalışmada; intratekal 5 mg hiperbarik %0.5 bupivakaine eklenen 12.5 mg ve 25 mg fentanilin, 5 mg hiperbarik %0.5 bupivakainle kıyaslandığında, bupivakaine fentanil ilavesinin blok seviyesi ve analjezi süresini klinik olarak anlamlı şekilde değiştirmediği, ancak yan etki sıklığını artırdığı sonucuna varıldı.
Abstract
The aim of this study was to investigate the effects of adding intrathecal fentanyl to low dose bupivacaine in lower extremity operations. Thirty ASA I or II class unpremedicated patients were randomly allocated into three identical groups. Spinal anesthesia was performed between L2-3 interspace with a 26 G atraumatic spinal needle in the lateral position operation side being dependent. Intrathecal 5 mg hyperbaric bupivacaine 0.5% (Group I), 5 mg hyperbaric bupivacaine 0.5% + 12.5 mg fentanyl (Group II) and 5 mg hyperbaric bupivacaine 0.5% + 25 mg fentanyl (Group III) were administered over 10 seconds. Patients were turned to supine position after keeping them in the lateral position for 10 minutes. Maximum sensorial and motor blockade levels of both of the extremities, times of sensorial blockade level regression to L1 dermatome and first analgesic requirements in the postoperative 24 hours, side effects and complications were recorded. Maximal sensorial blockade levels were at T11 (T8-L1), T9 (T3-12) and T10 (T4-L1) in groups I, II and III, respectively on the operation side. However, no sensorial blockade was observed on the contralateral extremity for all groups. During the operation, 3ş motor blockade was observed in 1, 4 and 6 patients in Group I, II and III, respectively on the operation side whereas, no motor blockade was observed on the contralateral extremity. There were no significant differences in the first analgesic requirements and regression of sensorial blockade to L1 dermatome times among the groups. The incidence of side effects such as; pruritis, shivering, nausea-vomiting and urinary retention were higher in group III. In the present study, addition of 12.5 and 25 mg fentanyl to 5 mg hyperbaric 0.5% bupivacaine did not change the block level and duration of analgesia siginificantly, but increased the incidence of side effects when compared to intrathecal 5 mg hyperbaric 0.5% bupivacaine alone.
