Yaşlı hastalarda kalça protezi cerrahisinde sürekli spinal, sürekli epidural ve izofluran-remifentanil anestezisinin karşılaştırılması

Özet
Amaç: Bölgesel anestezi yöntemleri ile kalça protezi cerrahisi geçirecek yaşlı hastalarda perioperatif istenmeyen olayların sıklığının azaldığı bildirilmiştir. Bu çalışmada, kalça protezi cerrahisi uygulanan yaşlı hastalarda sürekli spinal, sürekli epidural ve izofluran-remifentanil anestezisi yöntemlerinin hempdinamik parametreler, kan kaybı ve kan transfüzyon ihtiyacı açılarından karşılaştırılması amaçlandı. Yöntem: Kalça protezi cerrahisi geçirecek 60 yaş üzeri 45 hasta rast gele prospektif olarak çalışmaya alındı. Anestezi yöntemi olarak sürekli spinal anestezi grubunda (Grup SSA, n=15) spinal kateterle hiperbarik bupivakain; sürekli epidural anestezi grubunda (Grup SEA, n=15) epidural kateterle bupivakain ve izofluran-remifentanil anestezisi grubunda ise (Grup GA, n=15) indüksiyonda tiyopental, remifentanil ve idamede izofluran kullanıldı. İntraoperatif hemodinamik ölçümler, istenmeyen olaylar, kanama miktarı ve kan transfüzyon oranları kaydedildi. Bulgular: Grupların demografik ve preoperatif özellikleri benzerdi. Grup SSA'da intraoperatif hipotansiyona daha az rastlandı (p<0.03, her iki grup için). İntraoperatif komplikasyonlar açısından gruplar arasında anlamlı fark saptanmadı. Sonuç: Yaşlı hastalarda kalça protezi cerrahisinde genel ve epidural anestezi teknikleri ile karşılaştırıldığında sürekli spinal anestezi ile intraoperatif hipotansiyon sıklığı azalmıştır.
Abstract
Objective: It is reported that in the patients presenting for major orthopedic procedures, regional anesthesia, compared to general anesthesia reduces the perioperative complications and intraoperative blood loss. The aim of this study was to compare three different anesthetic techniques with respect to the intraoperative hemodynamic parameters, blood loss and transfusion requirements. Method: We enrolled 45 patients (>60 years) who were scheduled for hip arthroplasty surgery to this prospective study. Patients were randomly divided into three groups. In continuous spinal anesthesia group (Group SSA, n=15), hiperbaric bupivacaine was injected through a spinal catheter; in continuous epidural anesthesia group (Group SEA, n=15), isobaric bupivacaine was administered via an epidural catheter and in isoflurane-remifentanil anesthesia group (Group GA, n=I5), anesthesia was induced by thiopenthal and maintained with remifentanil and isoflorane in an equal mixture of O2 and air. Intraoperative hemodynamic measurements, complications, blood loss and transfusion requirements were recorded. Results: The demographic data and the preoperative characteristics of the patients were similar among the groups. SSA is associated with a lower incidence of intraoperative hypotension when compared with GA and SEA techniques during hip arthroplasty (p<0.03). Conclusion: Continuous spinal anesthesia, when compared to the general and continuous epidural anesthesia techniques reduces the incidence of the intraoperative hypotension.
