Özet
Gebelikte tümör oluşumu nadirdir, ortalama 1:1000 veya 1:2000 gebelikte görülür. Bu olgu ile T3 seviyesinde tümöre bağlıspinal kord hasarı bulunan ve sezeryan operasyonu planlanan gebenin anestezi yönetiminin sunulması amaçlandı. Otuziki yaşında, 30.gebelik haftasında spinal tümörü olan hastaya konsey sonucu (Kadın-Doğum, Beyin Cerrahi-Anesteziyoloji) sezeryan seksiyo kararıalındı. Bebeğin çıkımını takiben uterus posteriorunda ve batın içinde multipl tümöral oluşumlar tespit edildi ve tümör rezeksiyonuiçin operasyona devam kararı alındı. Peroperatif bir sorunla karşılaşılmadı. Operasyon sonunda kas gevşetici antagonisti uygulanarakhasta sorunsuz bir şekilde ekstübe edildi. Spinal kord hasarı; üriner sistem enfeksiyonları, dekübit ülserler, tromboflebit, anemi,azalmış respiratuvar rezerv ve otonomik hiperrefleksi gibi komplikasyonlara neden olabilen ciddi bir hastalıktır. Böyle durumlardadetaylandırılmış bir preoperatif değerlendirme ile hastaya uygun anestezi yöntemi seçimi ve yakın peroperatif takibin olasıkomplikasyonları önleyebileceği kanaatindeyiz.
Abstract
The development of tumours in pregnancy is rare, seen about 1:1000 or 1:2000 of pregnancies. Here, in this study we aimedto present the anesthesia management of the pregnant who has spinal injury due to a tumour at T3 level and planned for Caeseriansection. Caesarean section of the patient who is 32 year-aged, and in her 30 weeks of pregnancy with spinal tumor was decidedby council (Gynecology and Obstetrics, Neurosurgery, Anesthesiology). Following the delivery of the baby, at the posterior of theuterus and in the abdomen many tumoural formations were detected and for the resection of tumours the attendance of the operationwas decided. No problems were encountered peroperatively. At the end of the operation the patient was extubated without a problem byapplying the reversal of neuromuscular blockade. Spinal cord injury is a serious disease that can result in complications such asurinary tract infections, decubitis ulcers, thrombohlebitis, anemia, reduced respiratory reserve and hyperreflexia. In these situations,our opinion is, the election of an anesthesia management available for the patient with a detailed preoperative assessment andclose peroperative follow-up can prevent possible complications.
