ANESTEZİ YÖNÜNDEN ENDOSKOPİK ENDONAZAL TRANSSFENOİDAL HİPOFİZ CERRAHİSİ

Özet
Amaç: Günümüzde endoskopik endonazal transsfenoidal yöntem hipofiz tümörlerinin cerrahi tedavisinde çoğunluklakullanılmaktadır. Diğer nöroanestezi vakalarında olduğu gibi bu cerrahilerin anestezisi de özel yaklaşım gerektirmektedir. Bizimamacımız da hastanemizde sıkça yapılan bu cerrahilerin anestezik özelliklerini literatür eşliğinde sunmaktır.Yöntem: Etik kurul onayı alındıktan sonra retrospektif olarak hipofiz cerrahisi geçiren 380 hastanın bilgilerine ulaşılarak,demografik verileri, tümöre ait özellikler, ASA skoru, eşlik eden hastalıkları, anestezi süresi, cerrahi süre, Mallampati skoru,entübasyon güçlüğü olup olmadığı, monitörizasyon yöntemleri, anestezi yöntemleri, anestezik ilaçlar, kullanılan sıvı, kan ürünlerikaydedilmiş, peroperatif komplikasyonlar ve postoperatif yoğun bakım ihtiyacı da eklenmiştir. Bulgular: İkiyüzbir hasta sekretuar adenoma sahiptir. Yüzonsekiz hasta akromegalik olup bunların 5'inde zor havayolu ilekarşılaşılmış, ancak hastalar başarıyla entübe edilmişlerdir. Cushing hastalığı olan 62 hastanın sadece birinnde intraoperatifhipertansiyon görülmüş, 2 hasta da ise venöz damar yolu bulmakta zorluk çekilmiştir. Hastaların ikisinde postoperatif diabetesinsipidus, birinde postoperatif bulantı-kusma gelişmiştir. Genel olarak hastaların intraoperatif ve postoperatif komplikasyon oranı% 4.4 olmuştur.Sonuç: Hipofizer hormonların hiper ya da hiposekresyonunun sonuçları ve tümörün çevre dokuya kitle etkisi anesteziyiilgilendiren problemlere neden olabilir. Bu nedenle hipofiz tümörlü hastaların endokrinoloji, beyin cerrahisi, anestezinin dahilolduğu ekiple yönetilmesi uygundur. Operasyon öncesinde hastanın sistemik durumunun tespit edilmesi ve konsültasyonlarınıntamamlanması morbidite ve mortaliteyi önlemektedir
Abstract
Objective: Nowadays endoscopic endonasal transsphenoidal method is mostly used in the surgical treatment of pituitarytumors. As with other neuroanesthesia cases, anesthesia requires a special approach in this surgical cases. Our aim is using theliterature, to present the anesthetic properties of these surgical operations which are frequently performed in our hospital.Method: After approval of the ethics committee, 380 patients who had retrospective pituitary surgery were reached; demographicdata, characteristics of the lesion, ASA score, accompanying diseases, duration of anesthesia, duration of surgery, Mallampatiscore, intubation difficulty, monitorization methods, anesthetic drugs, bleeding condition , preoperative complications, andpostoperative intensive care unit were evaluated.Results: Two hundred and one patients have secretary adenoma. 118 patients were acromegalic, 5 of whom had difficultairways, but patients were successfully entubated. Of 62 patients with Cushing's disease, only 1 had intraoperative hypertensionand 2 patients had difficulty in finding a venous line. 1 of the patients had postoperative nausea and vomiting and 2 of them hadpostoperative diabetes insipidus. In general, the intraoperative and postoperative complication rates of the patients were 4.4%.Conclusion: The consequences of hyper- or hyposecretion of the hypophyseal hormones and the mass effect of the tumor onthe surrounding tissue may cause problems related to anesthesia. For this reason, pituitary tumor-based disease management isvery important and it is appropriate to manage between endocrinology, neurosurgery, and anesthesia. Detection of the systemiccondition of the patient before the operation and completion of consultations prevent morbidity and mortality
