Femur boyun kırıklarında 3 adet vida ile internal tespit: Sonuçlar ve komlikasyonlar

Özet
Amaç: İnternal tespit ile tedavi edilmiş femur boyun kırıklı hastaların ortalama 3 yıllık takip sonuçları ve komplikasyonları değerlendirildi. Çalışma planı: Haziran 2000 ile mayıs 2005 tarihleri arasında kliniğimize femur boyun kırığı tanısıyla başvuran ve internal tespit ile tedavi edilmiş 37 hastadan tespit materyali olarak 3 adet vida kullanılan ve en az 2 yıl düzenli takibi yapılabilen 26 (11 erkek,15 kadın; ort. yaş 39.8; dağılım 16-69) hastanın sonuçları incelendi. Sonuçlan etkilememesi için başka bölgelerinde kırıkları olan hastalar çalışmaya dahil edilmedi. Garden sınıflandırılmasına göre 4 hasta (%15) tip-1, 8 hasta (%31) tip-2, 6 hasta (%23) tip-3 ve 8 hasta (%31) tip-4 idi. Fonksiyonel sonuçlar UÇLA kalça skoruna göre değerlendirildi. Ortalama takip süresi 37 ay (dağılım 24-64 ay) idi. Bulgular: Kaynama elde edilen hastalarda ortalama kaynama süresi 5.5 ay (dağılım 3-12 ay) idi. Bir hastada ise kaynama olmadı ve ameliyat sonrası 15. ayda total kalça protezi yapıldı. Dört hastada femur başı avasküler nekroz (AVN) gelişti. İlk 24 saatte internal tespit yapılan hastalarda AVN oranı %9.5 iken, 24 saatten sonra tespit yapılan hastalarda bu oran %40 idi. AVN gelişen hastalardan birine total kalça protezi,diğerine bipolar protez yapıldı.AVN gelişen diğer 2 hastada ek bir cerrahi girişime ihtiyaç duyulmadı. Bir hastada ameliyat sonrası 35.günde implant yetmezliği gelişti ve bipolar protezle tedavi edildi. Onbeş (%58) hastada mükemmel, 5 (%19) hastada iyi, 5 (%19) hastada orta, 1 (%4) hastada kötü sonuç alındı. Çıkarımlar: Artroplasti yapılması için yaşı genç olan femur boyun kırıklı hastaların tedavisi acildir ve 3 vida ile internal tespit seçkin bir tedavi yöntemidir. Kırık özellikle ilk 24 saatte teda¬vi edilmiş ise hasta ve hekim açısından başarı şansı artacaktır.
Abstract
Objectives: To evaluate the results and complications of femoral neck fractures treated with internal fixation with three screws with an average follow-up of three years. Methods: Thirty seven patients operated upon between June 2000 and May 2005 for fractures of the femoral neck with three screws were identified as the study population. The final results of 26 patients (11 male, 15 female, with an average age of 39.8, range 16-69) that had a mini mum of two year follow-up (average 37 months, range 24-64 months) were evaluated. Those patients that had additional fractures were not included in the evaluation. Preoperatively based on Garden classification four patients (15%) were Type I, eight patients (31%) were type 2, six patients (23%) were type three and eight patients (31%) were type 4. Functional results were evaluated according to the UCLA hip score. Results: The average time to union was 5.5 months (range 3-12 months). One patient did not achieve union and a total hip replacement was done in the 15th postoperative month. Four patients had avascular necrosis (AVN) of the femoral head. The incidence of AVN in patients who had an operation within 24 hours of fracture was 9.5% while 40% in patients operated later than that. In patients with AVN one required a total hip replacement, another required treatment with bipolar hemiarthroplasty, the other two patients did not require further operations. One patient underwent bipolar hemiarthroplasty after screw pullout occurred on the 35th postopera tive day. Fifteen patients (58%) had excellent results, five (19%) had good, 5 (19%) had moder ate results and one (4%) had a poor result. Conclusion: Operative treatment with internal fixation using three screws in femoral neck frac tures in the first 24 hours after a fracture gives favourable results in young patients that are too young to be considered for arthroplasty.
