Özet
Giriş: Guillain-Barré Sendromu (GBS), periferik sinirlerin demiyelinizasyonu ve progresif kas güçsüzlüğüyleilerleyen bir immün sistem hastalığıdır. Ciddi seviyede etkilenmiş hastaların kliniğine solunum yetersizliğieklenebilir, hayatı tehdit eden mekanik ventilatör ihtiyacı ortaya çıkabilir ve otonomik disfonksiyon görülebilirOlgu: 23 yaşında, ek hastalığı olmayan, askerde “Difteri-Tetanoz Toksoid Aşı” uygulaması sonrası düşmetarifleyen erkek hasta, servis yatışının ardından yoğun bakım bölümümüze yatırıldı. Senkop ve düşme etiyolojisitarandı. GBS ön tanısıyla tetikleyici sebep araştırması yapılan hastada yalnızca difteri tetanoz toksoid aşı öyküsüilişkili bulundu. Yoğun bakımı yatışının ardından tetrapleji ve bulber tutulum gelişen hasta mekanik ventilasyondatakip edildi. IVIG ve pulse steroid tedavileri alan hasta yoğun bakım yatışının 18. gününde ekstübe edildi.Solunum fizyoterapisi ve CPAP uygulanmaya devam eden hasta, yoğun bakım yatışının 47. gününde nörolojiservisine taburcu edildi.Sonuç: Akut gelişen polinöropati hastalarında, difteri-tetanoz toksoid aşısının Guillain- Barré Sendromu’nutetikleyebileceği akla getirilerek tanıya yönelik tetkik ve tedavinin hızlı bir şekilde gerçekleştirilmesi, hastanınprognozu açısından önem taşımaktadır.
Abstract
Introduction: Guillain-Barré Syndrome (GBS) is an immune system disease that progresses with demyelination of peripheral nerves and progressive muscle weakness. Severely affected patients may develop life-threatening respiratory failure which requires mechanical ventilation. Autonomic dysfunction is also a mortal condition in the course of the disease. Case: A 23-year-old male patient, with no comorbid disease, described a fall one day after the application of "Diphtheria-Tetanus Toxoid Vaccine" in military duty. He was admitted to our intensive care unit on day 2 of his hospitalization. Syncope and fall etiology were questioned. The preliminary diagnosis was GBS and the diphtheria-tetanus toxoid vaccine was determined to be the only related trigger for this diagnosis.The patient developed tetraplegia and bulbar dysfunction on day 1 of admittance to the intensive care unit and mechanical ventilation was commenced. He received intravenous immunoglobulin and pulse steroid treatments. On day 18, he was extubated and Continuous Positive Airway Pressure (CPAP) and respiratory physiotherapy were İzmir Eğitim ve Araştırma Hastanesi Tıp Dergisi (Medical Journal of İzmir Hospital) 25 (2): 155-158, 2021 156 İzmir Eğitim ve Araştırma Hastanesi Tıp Dergisi (Medical Journal of İzmir Hospital) commenced. The patient was discharged to the neurology ward on the day 47 day of his admittance of the intensive care unit. Conclusion: In acutely developing polyneuropathy patients, it is important for the patient's prognosis to be carried out quickly with diagnostic tests and treatment, bearing in mind that diphtheria-tetanus toxoid vaccine may trigger Guillain-Barré Syndrome.
