Özet

Giriş: Canlı vericili böbrek nakli, alıcı için mevcut faydalarıyla birlikte donörler için de artmış risk faktörleribarındırmaktadır. Biz bu çalışmamızda nakil amacıyla donör nefrektomi yapılan bireylerin uzun dönem takipsonuçlarını ortaya koymayı amaçladık.Gereç ve Yöntem: Ocak 1998 ile Haziran 2019 tarihleri arasında kliniğimizde gerçekleştirilen canlı vericili böbreknakillerine ait ve en az 1 yıl takip süresi olan 297 donör çalışmaya alındı. Veriler hasta dosyalarından retrospektifolarak kaydedildi. Son kontrolde donörlere ait klinik ve laboratuar özellikler bağış öncesi dönemle karşılaştırıldı.Ayrıca,donörler son kontroldeki GFR değerlerine göre iki gruba ayrılarak (Grup 1: GFR ˂ 60 mL/dk/1.73 m² ,Grup 2 : GFR ˃ 60 mL/dk/1.73 m² ) karşılaştırıldı.Bulgular: Donörlerin ortalama takip süresi 47,4±44,3[12-215] ay, ortalama yaşı 47.3 ± 11.4 yıldı. Kadın donörlerinsayısı 148 (49,8 %)di. Son kontrolde donörlere ait HT ve obezite oranı nefrektomi öncesi dönemlekarşılaştırıldığında anlamlı şekilde artış olduğu tespit edildi(6.3% vs 3.3%, 30.3% vs 21.5). Takip sırasında 79(26,6%) olguda GFR ˂ 60 mL/dk/1.73 m² (Grup 1) iken 218 (73,4%) olguda GFR ˃ 60 mL/dk/1.73 m² (Grup 2) idi.Gruplar karşılaştırıldığında 1. Gruptaki olguların daha yaşlı (52.3 ± 10 vs 45.5±11 yıl, p=0.001) ve nefrektomiöncesi dönemde daha yüksek hipertansif donör oranına( 6% vs 4%, p=0.01) ve bazal ürik asit seviyelerine sahip(5±1.4 vs 4.5±1.4, p=0.003) oldukları görüldü.Sonuç: Donör nefrektomi yapılan bireylerin uzun dönem takiplerinde HT ve obezite insidansı artış göstermektedir.GFR’de düşüş olmasına rağmen, son dönem böbrek yetmezliği (SDBY) riski düşüktür.

Abstract

Introduction: Living-donor kidney transplantation has increased risk factors for donors despite benefits for the recipients. In this study, we aimed to evaluate the long-term follow-up results of living kidney donors. Material and Method: Living donors of kidney transplants between January 1998 and June 2019 were retrospectively evaluated. 297 cases were included in the study. Donors with less than 1 year of follow-up were excluded. At the last control, the clinical and laboratory characteristics of the donors were compared with the predonation period. The participants were divided into to groups according to last control GFR value (Group 1: GFR ˂ 60 mL/dk/1.73 m², Group 2 : GFR ˃ 60 mL/dk/1.73 m² ). Results: In the included 297 donors, mean follow-up in months ± SD (range) was47,4±44,3[12-215]. The mean age in years ± SD (range) was47.3 ± 11.4 and 49,8 % of donors were female.At the last control, the hypertension(HT) and obesity rate of donors were found to be significantly increased compared to the prenephrectomy period (6.3% vs 3.3%, 30.3% vs 21.5). During follow-up, GFR was ˂ 60 mL / min / 1.73 m² (Group 94 İzmir Eğitim ve Araştırma Hastanesi Tıp Dergisi (Medical Journal of İzmir Hospital) 1) in 79 (26.6%) cases, and GFR was ˃ 60 mL / min / 1.73 m² (Group 2) in 218 (73.4%) cases. Compared with counterparts, the patients in the Group 1 were older (52.3 ± 10 vs 45.5 ± 11 years, p = 0.001) and had higher hypertension rate (6% vs 4%, p = 0.01) and basal uric acid levels in the pre-nephrectomy period (5 ± 1.4 vs 4.5 ± 1.4, p = 0.003). Conclusion: The incidence of HT and obesity has increased in the living kidney donors at the follow-up. Although there is a decrease in GFR, the risk of end stage renal failure (ESRD) is low.
