Özet
Nazal polipozis (NP) burun ve paranazl sinüslerin sık görülen benign leyonlanndan birisidir. Sık görülmesine ve ko-lay tanınmasına rağmen, etyopatogenezinin tam olarak ortaya koyulamaması ve tedavisindeki güçlükler nedeniyle tüm dün-yada olduğu gibi ülkemizde de önemini korumaktadır. Bu çalışmada, Şubat 1991 ile Ağustos 1996 tarihleri arasında Messerk-linger tekniği ile endoskopik sinüs cerrahisi (ESC) uygulanan 284 nazal polipozis olgusu değerlendirilmiştir. Hastaların; 11 'i (%3,9) evre-1,54'ü (%19.0) evre-2,102'si (%35,9) evre-3 ve 117'si (%41.2) evre-4 olup ve en büyük grubu evre-4 hastalar oluşturmuştur. Hastaların 189'unda (%66.5) anatomik varyasyon, 100'ünde (%35,2) alleriji ve/veya astım ve/veya ASA sensitivitesi (88'inde allerji -%31-, 35'inde astım -12,3-, 12 'sinde ASA sensitivitesi -%4,2-) mevcuttu. 92 hasta (%32.4) da-ha önce öpere edilmişti. Postoperatif dönemde ortalama izlem süresimiz 30 aydır ve hastaların semptomlarındaki sübjektif dü-zelmeler değerlendirilerek genel başarı oranımız %80,6 olarak belirlenmiştir. Serimizde majör komplikasyon oranımız %1,1 ve minör komplikasyon oranımız %22,5'dir.
Abstract
Nasal polyposis is one of the most common benign lesions of the paranasal sinuses. Although they are frequ-ently seen and easily recognised, they are still a world wide important problem in otorhinolaryngology due to liner indefinite ethiopathology and difficulty of treatment. In this study 284 nasal polyposis cases on whom endoscopie sinus surgery was per-formed between February 1991 and August 1996 with teh Messerklinger technique were evaluated. Patients were staged as 11 (3,9 %) stage-1; 54 (19,0 %) stage-2; 102 (35,9%) stage -3; 117 (41,2%) stage-4 and as can be seen the largest group was in stage-4. Of the 284 patients, 189 (66,5%) had an anatomic variation, 100 (35,2%) had allergy and/or asthma and/or ASA sensitivity (88 had allergy -31,0%-, 35 had asthma -12,3%-, 12 had ASA sensitivity -4,2%-), and 92 (32,4%) of the patients were previously operated on. Our average postoperative follow up was 30 months. Our general success rate was found to be 80,6% upon evaluating the subjective improvements in the patients' symptoms in the postoperative period. The major and minor complication rates in our series were, 1,1% and 22,5% respectively.
