Özet

Nazofarenksin (NF) incelenmesi anatomik yapısından dolayı oldukça güçtür. Ancak NF incelenmesi, fiberoptik en-doskopilar ile kitle veya kanserden şühelenilen olgularda topikal anestezi altında kolaylıla yapılabilmektedir. Ayrıca incele¬me esnasında bir forseps yardımıyla nazofarenksden tanı için yeterli biyopsi almak da mümküdür. Bu çalışma, kilinik olarak nazofarenks kanserinden (NFK) şüphelenilen veya rijid endoskop ile NF'de kitle saptanan ve ortalama yaşları 48.2 olan 31 olguda yapıldı. Olgulardan 18'sinde (%58) seröz otit, 7'sinde (% 22.57 boyunda kitle ve 5'inde (% 16.1) burun tıkanıklığı vardı: Endoskop ile topikal anestezi altında görüntülenerek 204si kitleden ve 11'i kör NF biyopsisi alındı. Endoskopik incele¬me ve biyopsi almaya bağlı bir komplikasyon olmadı. Histopatolojik incelemede 9 (%29) olguda NFK, 17 (% 54.8) olguda lenfoid hîperplazi, 4 (% 12.9) olguda normal doku ve 1 (% 3.2) olguda ise pseudokist saptandı. Kanser saptanan olgular on¬koloji merkezine gönderildi. NF'in kanser ve diğer kitlelerinin incelenmesinde endoskopik incelemenin kolay uygulanan, topi¬kal anestezi altıda biyopsi almaya izin veren erken bir tanı yöntemi olabileceği sonucuna varıldı.

Abstract

Nasopharynx (NP) is anatomically difficult to examine adequately. This procedure can be performed quickly and easly with a fiberoptic endoscope on the patients suspected with nasopharynx (NPC). It gives an exellent view ofNP and it is possible to obtain biopsies from NP under local anaesthesia. We have peiformed flbreoptic nasopharyngoscopy using rigid endoscope to examine 31 patients with nasopharyngeal mass and clinically suspected NPC. Their avarage ages were 48.2. Eghteen of 31 patients (% 58) had serous otitis media, 7 (% 22.5) had neck maas and 5 (% 16.1) had nasal obstructi¬ons. Using endoscope and small ethmoit forseps, blind biopsies were obteinedfrom 11 patients and from obvious lesions of 20 patients under topical anaestesia. We have not encountered any complication with this procedure. Eight of 30 patients (% 29) were diagnosed NPC, 4 (% 12.9) were normal, 17 (% 54.8) lymphoit hyperplasia and 1 (% 3.2) was pseudocystic lesion. The patients with NPC were treated by radiation in an oncology center. Our experience showed that the fiberoptic endosco-pic nasopharyngoscopy is a valuable instrument for examination of the NP and obtaining biopsies. Therfore, it can play more important role in detecting early NPC and other lesions.
