Özet
Çalışma, kliniğimize başvuran maksillofasial fraktürlü hastaların retrospektif analizini sunmaktadır. 268 hastanın yaş ortalaması 25.8 18 ve erkek-bayan oranı 17:1 idi. Düşme ve çarpma fraktürlerin en sık nedeniydi. 268 hastanın 249'u (% 92.9) izole, 19'u (% 7.1) multipl kemik fraktürlerine sahipti. İzole fraktürlerin 173'ü (% 69.5) nazal kemikte, 62'si (%24.9), mandibulada, 8'i (%3.2) alveolerproseste, 2'si (%0.8) orbitada, l'i (%0.4) zigomatik kemikte, l'i (%0.4) maksilla- da, l'i (%0.4) frontal kemikte ve l'i (%0.4) sert damaktaydı. 19 multipl fraktürlü hastanın 16 'sında (%84.2) orbita, 7'sinde (% 36.8) mandibula, 7'sinde (%36.8) zigomatik kemik, 6'sında (% 31.6) maksilla, 6'sında (%31.6) nazal kemik, 2'sinde (% 10.5) frontal kemik ve l'inde (% 5.3) sert damak fraktürü tespit edildi. Tedaviyi kabul etmeyen izole mandibula fraktürlü 1 hâsta ve kırık hatlarında ayrılma olmayan 3 alveoler ark fraktürlü hasta dışında, tüm olgular açık redüksiyon ve internal fiksasyon ya da kapalı redüksiyon ve ekstemal fiksasyon ile tedavi edildi. Sık görülen komplikasyonlar yara enfeksiyonu, ma- loklüzyon, kanama, fasial sinir paralizisi idi. 4 hastaya acil trakeotomi açıldı. Maksillofasial fraktürler sıklıkla kulak burun boğaz & baş boyun cerrahları ve plastik ve rekonstrüktif cerrahlar için sorun yaratabilirler. Bunun nedeni, maksillofasial bölgenin görme, konuşma, çiğneme, yutma ve solunum gibi önemli vücut fonksiyonları ile ilgili olmasının yanışım estetik açıdan önemi olması ve anatomisinin karmaşıklığıdır. Dolayısıyla bu bölgedeki fraktürlerin tedavisi dikkatle planlanmalı¬ dır.
Abstract
This study presents the retrospective analysis of maxillofacial fracture cases admitted to our Otolaryngology Clinic. The average age and the male to female ratios of 268 patients were 25.8 18 and 17:1respectively.: The most common causes öf fractures were falling down and crush injuries. Of 268 patients, 249 (92.9 %) had isolated fractures and 19 (7.1 %) had multiple fractures. The localizations of 249 isolated fractures were as fallows: 173 (69.5%) .nasal bone, 62 (24.9 %) mandible, 8 (3.2 %) alveolar bone, 2 (0.8 %) orbit, 1 (0.4 %) zygomatic bone, 1(0.4 %) maxilla, 1(0.4%) frontal bone and 1 (0.4 %) hard palate. 19 multiple fractures were localized as below: 16 (84.2 %) orbit, 7 (36.8%) mandible, 7 (36.8%) zygo¬matic bone, 6(31.6 %) maxilla, 6 (31.6 %) nasal bone, 2 (10.5 %) frontal, 1 (5.3 %) hard palate. Except 3 patients with alveo¬ler bone fracture which fracture lines did not show seperation and 1 patient with isolated mandible fracture who refused tre¬atment, all patients were treated by open reduction and internal fixation or closed reduction and external fixation. Common complications were wound infection, maloclusion, upper respiratory tract obstruction, bleeding and facial nerve paralysis. Emergency tracheotomy was performed in four patients. Maxillofacial fractures, frequently, create problems for ear nose throat & head and neck surgeons and also for plastic and reconstructive surgeons. These problems take origin from the ana¬tomic complexity and importance for esthetics and body functions -including vision, speech, mastication, deglutition, and ventilation-of this region. Consequently surgical approaches to this region need to be planned very carefully.
